基  金:蔗糖铁注射液加速给药方案治疗缺铁性贫血的有效性与安全性研究(2024-M-5)。

摘  要:目的评价蔗糖铁加速补铁方案治疗继发于各种病因的缺铁性贫血的有效性与安全性。方法回顾性分析了某三级医院2021年1月1日至2022年6月30日期间接受蔗糖铁(Iron sucrose,IS)补充治疗的住院患者病历。通过回顾IS输注期间任何不良反应的医疗记录以及输注前后肝功能、肾功能和血清磷检验参数来评估安全性;通过检测输注前后血红蛋白、网织红细胞绝对计数和铁参数来评估疗效。结果研究期间,共纳入59例患者,加速组24例,常规组35例。补铁后两组血红蛋白(Hb)水平均较补铁前明显升高(P<0.001),补铁后两组的Hb比较无统计学差异〔加速组(89.38±12.85)g·L^(-1);常规组(93.46±11.79)g·L^(-1),P=0.213〕,两组虽有较高的低磷血症发生率,但无临床症状和其它不良反应报告。加速组的住院天数明显短于常规组(P<0.001)。结论IS加速补铁方案治疗多病因缺铁性贫血安全有效,可显著提高Hb水平。此外,它在缩短住院时间方面具有显著优势。OBJECTIVE To evaluate the efficacy and safety of an accelerated iron sucrose dosing regimen for the treatment of iron deficiency anemia secondary to various etiologies.METHODS A retrospective analysis of medical records was performed on hospitalized patients who received iron sucrose(IS)supplementation between January 1,2021 and June 30,2022 at a tertiary hospital.Safety was evaluated by reviewing medical records for any adverse reactions during IS infusion and parameters of liver function,renal function and serum phosphorus tests before and after infusion.Efficacy was assessed by examining the absolute reticulocyte count,hemoglobin,and iron parameters before and after infusion.RESULTS A total of 59 patients were enrolled during the study period,with 24 in the accelerated group and 35 in the conventional group.Significant increases in hemoglobin(Hb)levels were observed in both groups after iron supplementation compared to pre-treatment values(P<0.001).There was no statistical difference in Hb levels between the two groups post-treatment〔(89.38±12.85)g·L^(-1) in the accelerated group;(93.46±11.79)g·L^(-1) in the conventional group,P=0.213〕.Although the incidence of hypophosphatemia was higher in both groups,no clinical symptoms or other adverse reactions were reported.CONCLUSION Intensive iron supplementation is both safe and effective in managing multi-cause iron deficiency anemia,markedly elevating hemoglobin levels.Moreover,it exhibits a significant advantage in abbreviating hospitalization duration.
