摘  要:目的探讨血清乳酸脱氢酶/清蛋白(LDH/ALB)、肿瘤坏死因子相关凋亡诱导配体(TRAIL)、线粒体衍生肽(MOTS-c)在儿童急性病毒性下呼吸道感染中的水平,并探讨三者对疾病的诊断价值以及对病情程度的评估价值。方法选择2019年1月至2023年1月就诊于该院的196例急性病毒性下呼吸道感染患儿作为急性病毒组,根据患儿的病情严重程度将急性病毒组分为轻中度组(138例)和重度组(58例)。另选取同期在该院体检健康的儿童182例作为对照组。比较各组血清LDH/ALB,以及TRAIL、MOTS-c水平。采用多因素Logistic回归分析重度急性病毒性下呼吸道感染的影响因素;采用受试者工作特征(ROC)曲线分析血清LDH/ALB、TRAIL、MOTS-c对疾病的诊断价值和病情严重程度的评估价值。结果急性病毒组血清LDH/ALB、TRAIL水平高于对照组,且重度组高于轻中度组比,差异均有统计学意义(P<0.05)。急性病毒组血清MOTS-c水平低于对照组(P<0.05),且重度组低于轻中度组(P<0.05)。血清LDH/ALB、TRAIL水平升高是重度急性病毒性下呼吸道感染的危险因素(P<0.05),血清MOTS-c水平升高是保护因素(P<0.05)。血清LDH/ALB、TRAIL、MOTS-c联合诊断儿童急性病毒性下呼吸道感染的曲线下面积(AUC)优于各指标单独诊断(P<0.05);血清LDH/ALB、TRAIL、MOTS-c联合评估患儿病情严重程度的AUC优于各指标单独评估(P<0.05)。结论急性病毒性下呼吸道感染患儿血清LDH/ALB、TRAIL水平均明显上升,MOTS-C水平明显下降,三者联合检测对诊断儿童急性病毒性下呼吸道感染及评估病情严重程度有较高价值。Objective To investigate the expression of serum lactate dehydrogenase/albumin(LDH/ALB),tumor necrosis factor-related apoptosis-inducing ligand(TRAIL)and mitochondria-derived peptide(MOTS-c)in children with acute viral lower respiratory tract infections,and to evaluate the diagnostic value of the above three indicators for the diagnosis of the disease as well as for the assessment of the severity of the disease.Methods A total of 196 children diagnosed with acute viral lower respiratory tract infections in the hospital from January 2019 to January 2023 were selected as the acute virus group,which was subdivided into the mild-moderate group(138 cases)and the severe group(58 cases)according to the severity of the disease.Additionally,182 healthy children who underwent physical examinations during the same period were included as the control group.Serum LDH/ALB,TRAIL and MOTS-c levels were compared between the groups.Multivariate Logistic regression was used to analyze the influencing factors of severe acute viral lower respiratory tract infections,and the receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum LDH/ALB,TRAIL and MOTS-c for the diagnosis of the disease and assessment of the disease severity.Results Serum LDH/ALB and TRAIL levels were higher in the acute virus group than in the control group,while those in the severe group were higher than those in the mild-moderate group,and the differences were statistically significant(P<0.05).Serum MOTS-c level was lower in the acute virus group than that in the control group(P<0.05),and which was lower in the severe group than the mild-to-moderate group(P<0.05).Elevated serum LDH/ALB and TRAIL levels were risk factors for severe acute viral lower respiratory tract infection(P<0.05),while elevated MOTS-c level was a protective factor(P<0.05).The area under the curve(AUC)for the combined diagnosis of acute viral lower respiratory tract infection in children by serum LDH/ALB,TRAIL and MOTS-c was superior to that of each index al
