摘　　要：目的:探讨黄色肉芽肿性胆囊炎(XGC)多层螺旋CT(MSCT)表现,以提高该病术前诊断准确率。方法:回顾性分析2017年3月—2023年12月太仓市中医医院收治的7例XGC患者MSCT表现。所有患者的资料完整可靠,在术前进行了CT诊断并在术后进行病理诊断,并复习相关文献。结果:7例XGC均表现为胆囊壁不同程度增厚,增强扫描增厚的胆囊壁呈轻~中度强化,6例增厚的胆囊壁内见条片状、结节状低密度灶,增强扫描低密度灶均无明显强化;5例囊内壁光整,黏膜线连续;2例黏膜面毛糙,不完整;7例胆囊内见均见结石,2例累及肝脏,2例与周围肠管粘连、分界欠清。结论:XGC的MSCT表现及病理具有一定的特征性,MSCT检查有助于XGC的术前诊断。Objective To explore the multi-slice spiral CT(MSCT)findings of xanthogranulomatous cholecystitis(XGC)to improve preoperative diagnostic accuracy.Methods Retrospective analysis of MSCT findings in 7 XGC patients admitted to Taicang Hospital of Traditional Chinese Medicine from March 2017 to December 2023.All patient data were complete and reliable.CT diagnosis was performed before surgery and pathological diagnosis was performed after surgery,and relevant literature was reviewed.Results All of the 7 cases of XGC indicated a thickened-wall gallbladder in varying degrees and an enhanced scan of the thickened-wall gallbladders suggested an intensification from mild to moderate,of which 6 cases indicated intramural hypoattenuated nodules or bands.No obvious intensification was seen in the enhanced scan of hypoattenuated nodules.Of the 7 cases,5 cases were found with an even cystic inner wall and consecutive mucosal lines;while 2 cases were observed with rough and uncompleted mucous.Gallbladder calculi were found in all of the 7 cases,of which 2 cases involved the liver;2 cases were found with adhesion to the adjacent intestines and blurred boundaries.Conclusion The MSCT manifestations and pathology of XGC have certain characteristics,and MSCT examination is helpful for preoperative diagnosis of XGC.
