摘 要:目的:探讨终末期肾病(ESRD)患者在不同血液净化模式下,应用常规超声心动图评价左心室射血分数(LVEF)正常患者的左心室重构。方法:选取2022年1月—6月兰州市第一人民医院肾病内科血液净化中心收治的ESRD LVEF正常患者60例。其中30例为血液透析(HD)组,30例为高通量血液透析(HFHD)组。比较患者透析治疗6个月后,两组研究对象心脏彩超左心室腔室大小、左心室构型等指标差异。结果:HFHD组主动脉根部内径(AOd)、AOd/体表面积(BSA)、左心房内径(LAd)、LAd/BSA、左心室舒张末期内径(LVEDd)、LVEDd/BSA、左心室收缩末期内径(LVESd)、LVESd/BSA、左心房容积(LAV)、心房容积指数(LAVI)、左心室舒张末期容积(LVEDV)、LVEDV/BSA、左心室收缩末期容积(LVESV)、LVESV/BSA、室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、相对室壁厚度(RWT)、左心室质量(LVM)、左心室质量指数(LVMI)值均小于HD组,HD组左心室短轴缩短率(LVFS)、左心室射血分数(LVEF)值小于HFHD组,差异均有统计学意义(P<0.05)。左心室构型分型:HD组构型正常(NG)数(12例)少于HFHD组正常数(22例);HD组构型异常数18例[向心性重构(CR)1例,离心性肥厚(EH)10例,向心性肥厚(CH)7例]多于HFHD组的8例(离心性肥厚5例,向心性肥厚3例)。结论:ESRD患者选择HFHD方式,可明显地降低左心室重构发生率。Objective To investigate the use of conventional ultrasonocardiography to evaluate left ventricular remodeling in patients with normal left ventricular ejection fraction(LVEF)in patients with end-stage renal disease(ESRD)under different blood purification modes.Methods From January 2022 to June 2022,60 patients with normal LVEF of ESRD were selected from the Blood purification Center of Nephropathy Department of the First People's Hospital of Lanzhou City.Among them,30 cases were hemodialysis(HD)group and 30 cases were high-flux hemodialysis(HFHD)group.The differences of left ventricular chamber size and left ventricular configuration between the two groups were compared after 6 months of dialysis treatment using ultrasonocardiography.Results Aortic root diameter(AOd),AOd/body surface area(BSA),left atrial diameter(LAd),LAd/BSA,left ventricular end diastolic diameter(LVEDd),LVEDd/BSA,left ventricular end systolic diameter(LVESd),LVESd/BSA,left atrial volume(LAV),atrial volume index(LAVI),left ventricular end diastolic volume(LVEDV),LVEDV/BSA,left ventricular end systolic volume(LVESV),LVESV/BSA,interval thickness(IVST),left ventricular posterior wall thickness(LVPWT),relative wall thickness(RWT),left ventricular mass(LVM)and left ventricular mass index(LVMI)values in HFHD group were smaller than the HD group;left ventricular short axis shortening(LVFS)and left ventricular ejection fraction(LVEF)were lower than those in HFHD group,the differences were statistically significant(P<0.05).Left ventricular configuration classification:the number of normal configuration in HD group(12 cases)was less than that in HFHD group(22 cases),and the number of abnormal configuration in HD group(18 cases,including 1 case of concentric remodeling,10 cases of eccentric hypertrophy,7 cases of concentric hypertrophy)was more than that in HFHD group(8 cases,including 5 cases of centrifugal hypertrophy and 3 cases of centripetal hypertrophy).Conclusion ESRD patients select the HFHD mode,can significantly reduce the incidence of left ven
