慢性阻塞性肺疾病急性加重期合并支气管扩张患者P选择素与降钙素原、凝血功能的相关性研究

摘　　要：目的探究慢性阻塞性肺疾病急性加重期(AECOPD)合并支气管扩张患者外周血P选择素水平与降钙素原(PCT)、凝血功能及病情严重程度的关系。方法前瞻性选取2023年3—10月牡丹江医科大学附属红旗医院收治的40例AECOPD合并支气管扩张患者作为观察组,40例未合并支气管扩张的AECOPD患者作为病例对照组,以同期30例健康体检者作为健康对照组。检测各组研究对象外周血P选择素、PCT、D-二聚体、纤维蛋白原(Fib)水平,分析P选择素与PCT、D-二聚体、Fib及AECOPD合并支气管扩张患者病情严重程度的相关性。结果观察组P选择素、PCT、D-二聚体、Fib水平明显高于病例对照组及健康对照组,且病例对照组明显高于健康对照组,差异有统计学意义(P<0.001)。观察组、病例对照组外周血P选择素水平与PCT、D-二聚体、Fib水平均呈正相关(P<0.05),且P选择素、PCT、D-二聚体、Fib水平与AECOPD合并支气管扩张患者病情严重程度均呈正相关(P<0.05)。结论AECOPD合并支气管扩张患者外周血P选择素水平与PCT、D-二聚体、Fib水平呈正相关,且P选择素在评估病情严重程度方面具有一定临床价值。Objective To investigate the relationship between peripheral blood P-selectin levels and procalcitonin(PCT),coagulation function and disease severity in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)combined with bronchiectasis.Method Forty patients with AECOPD combined with bronchiectasis admitted to Hongqi Hospital affiliated to Mudanjiang Medical College from March to October 2023 were selected as the observation group,and 40 patients with AECOPD not combined with bronchiectasis were selected as the case control group,and 30 healthy medical check-ups during the same period were used as the healthy control group.The peripheral blood levels of P-selectin,PCT,D-dimer and fibrinogen(Fib)were determined in the subjects of each group,and the correlation between P-selectin and PCT,D-dimer,Fib and the severity of disease in patients with AECOPD combined with bronchiectasis was analysed.Result The levels of P-selectin,PCT,D-dimer and Fib in the observation group were significantly higher than those in the case control and healthy control groups,and the case control group was significantly higher than the healthy control group,and the difference was statistically significant(P<0.001).Peripheral blood P-selectin levels and PCT,D-dimer and Fib levels were positively correlated in the observation group and the case control group(P<0.05),and the levels of P-selectin,PCT,D-dimer and Fib were positively correlated with disease severity in patients with AECOPD combined with bronchiectasis(P<0.05).Conclusion Peripheral blood P-selectin levels were positively correlated with PCT,D-dimer and Fib levels in patients withAECOPD combined with bronchiectasis,and P-selectin has some clinical value in assessing disease severity.
