摘 要:目的观察孕中晚期健康胎儿心脏二维斑点追踪成像(2D-STI)参数与孕周的相关性。方法前瞻性对279胎健康单胎胎儿行超声心动图检查,根据孕周将其分为A组(孕20~23^(+6)周,n=49)、B组(孕24~27^(+6)周,n=112)、C组(孕28~31^(+6)周,n=63)、D组(孕32~35^(+6)周,n=30)及E组(孕周≥36周,n=25);采用2D-STI获取胎儿左心室(LV)射血分数、LV和右心室(RV)面积变化率、LV和RV整体径向应变(GRS)、LV和RV心肌和心内膜整体纵向应变(MyoGLS,EndoGLS)、LV心肌和心内膜整体周向应变(MyoGCS,EndoGCS),比较组间差异,并分析与孕周的相关性。结果5组胎儿LV EndoGLS差异有统计学意义(P=0.044)。胎儿LV GRS及RV GRS与孕周均呈正相关(r=0.165、0.122,P均<0.05),而LV EndoGLS绝对值与孕周呈负相关(r=-0.133,P=0.026)。结论孕中晚期健康胎儿LV GRS及RV GRS与孕周均呈正相关,而LV EndoGLS绝对值与孕周呈负相关。Objective To observe the correlations of two-dimensional speckle tracking imaging(2D-STI)parameters of healthy fetal heart and gestational age in middle and late pregnancy.Methods Fetal echocardiography was prospectively performed in 279 healthy singleton fetuses in middle and late pregnancy.The fetuses were divided into group A(20-23^(+6)weeks of pregnancy,n=49),B(24-27^(+6)weeks of pregnancy,n=112),C(28-31^(+6)weeks of pregnancy,n=63),D(32-35^(+6)weeks of pregnancy,n=30)and group E(≥36 weeks of pregnancy,n=25)according to gestational age.Fetal left ventricle(LV)ejection fraction,LV and right ventricle(RV)area change fraction,LV and RV global radial strain(GRS),LV and RV myocardium and endocardium global longitudinal strain(MyoGLS,EndoGLS),LV myocardium and endocardium global circumferential strain(MyoGCS,EndoGCS)were obtained with 2D-STI and compared among 5 groups,and the correlations with gestational age were analyzed.Results Significant differences of fetal LV EndoGLS was found among 5 groups(P=0.044).Fetal LV GRS and RV GRS were positively correlated with gestational age(r=0.165,0.122,both P<0.05)in middle and late pregnancy,while the absolute value of LV EndoGLS was negatively correlated with gestational age(r=-0.133,P=0.026).Conclusion LV GRS and RV GRS were positively correlated with gestational age in healthy fetuses in middle and late pregnancy,while the absolute value of LV EndoGLS was negatively correlated with gestational age.
