黄连温胆汤合逍遥散加减方治疗围绝经期睡眠障碍的效果观察

摘　　要：目的:探讨黄连温胆汤合逍遥散加减方治疗围绝经期睡眠障碍的效果。方法:选取2022年3月至2023年9月漳州正兴医院收治的接受围绝经期睡眠障碍治疗患者84例作为研究对象,按照随机数字表法分为对照组和观察组,每组42例。对照组给予戊酸雌二醇联合佐匹克隆治疗,观察组在对照组的基础上给予黄连温胆汤合逍遥散加减方治疗。采用匹兹堡睡眠质量指数(PSQI)比较2组患者干预前后睡眠质量的变化,采用围绝经期综合征生命质量评定量表(MENQOL)对治疗前、治疗4周后2组生命质量进行评估,比较2组患者神经递质水平的差异,包括5-羟色胺(5-HT)、多巴胺(DA)、γ-氨基丁酸(GABA)指标水平,并比较2组患者治疗有效率。结果:干预后,观察组治疗后总有效率显著高于对照组,差异有统计学意义(P<0.05);干预后,观察组PSQI量表评分显著低于对照组,差异均有统计学意义(均P<0.05);干预后,观察组MENQOL量表4个不同维度评分显著低于对照组,差异均有统计学意义(均P<0.05);干预后,观察组5-HT表达水平显著高于对照组,DA、GABA指标显著低于对照组,差异均有统计学意义(均P<0.05)。结论:黄连温胆汤合逍遥散加减方治疗围绝经期睡眠障碍的效果满意,可有效提高患者睡眠质量与生命质量,也能调节神经递质水平。Objective:To investigate the effect of Huanglian Wendan Decoction and Xiaoyao SAN Jiajie prescription on perimenopausal sleep disorders.Methods:A total of 84 patients receiving treatment for perimenopausal sleep disorders in our hospital from March 2022 to September 2023 were selected as research objects,and were divided into control group and observation group according to random number table method,with 42 cases in each group.The control group was treated with estradiol valerate combined with Zopiclone,and the observation group was treated with Huanglian Wendan Decoction combined with Xiaoyao SAN on the basis of the control group.Pittsburgh Sleep Quality Index(PSQI)was used to compare the changes of sleep quality before and after intervention between the two groups.Perimenopausal Syndrome Quality of Life Rating Scale(MENQOL)was used to evaluate the quality of life between the two groups before treatment and 4 weeks after treatment,and the differences in neurotransmitter levels between the two groups were compared.The levels of 5-hydroxytryptamine(5-HT),dopamine(DA),gamma-aminobutyric acid(GABA)were included,and the efficacy of treatment was compared between the two groups.Results:After intervention,the total effective rate of the observation group was significantly higher than that of the control group,and the difference between the two groups was statistically significant(P<0.05).After intervention,the PSQI score of the observation group was significantly lower than that of the control group,and the difference between the two groups was statistically significant(P<0.05).After intervention,the scores of 4 different dimensions of MENQOL scale in the observation group were significantly higher than those in the control group and the differences between the two groups were statistically significant(P<0.05).After intervention,the expression level of 5-HT in the observation group was significantly higher than that in the control group,and the indexes of DA and GABA were significantly lower than those in the control
