某院抑郁伴睡眠障碍患者镇静催眠类药物的用药状况及合理性

摘　　要：目的:探讨某院2023年抑郁伴睡眠障碍患者应用镇静催眠类药物的用药状况及合理性分析,为抑郁伴睡眠障碍的临床用药提供指导。方法:选取2023年1月至2023年12月淄博市精神卫生中心开具的抑郁伴睡眠障碍患者应用镇静催眠类药物治疗的70893张处方作为研究对象,分析这些处方的人群特点分布、镇静催眠类药物的用药状况、用药不合理情况。结果:研究结果显示,40~49岁患者的占比相对较多,占比为20.04%;≤19岁、20~29岁、30~39岁患者男性比例相对较高,40~49岁患者男女比例相当,而50~59岁、60~69岁、70~79岁的女性比男性比例较高,>79岁的患者中男性比例又增高;本研究中所选取的70893的处方中,应用最多的为阿普唑仑片,占比为36.29%;其次为佐匹克隆片、氯硝西泮片、艾司唑仑片,占比分别为15.93%、15.28%、10.02%;本研究中选取的70893处方中,1315张不合理处方,占比为1.90%。其中不合理处方主要包括联合用药不适宜、药物选用不适宜、用法用量不适宜、用药疗程不适宜、给药时机不适宜、超说明书用药,占比分别为30.27%、23.35%、22.21%、11.63%、8.37%、4.18%。结论:研究显示,该院提供抑郁伴睡眠障碍患者治疗常用药品为阿普唑仑片、佐匹克隆片、氯硝西泮片、艾司唑仑片,大部分药物在使用中存在合理性,少数存在不合理现象,需加强用药指导。Objective:To investigate the use of sedative and hypnotic drugs in patients with depression and sleep disorders in a hospital in 2023 and to provide guidance for clinical use of depression and sleep disorders.Methods:From January 2023 to December 2023,70893 prescriptions of sedative and hypnotic drugs were collected from a hospital for depression patients with sleep disorders.The distribution of these prescriptions,the use of sedative and hypnotic drugs,and the unreasonable use of drugs were analyzed.Results:The study results showed that the proportion of patients aged 40~49 was relatively large,accounting for 20.04%;The proportion of males in patients aged≤19 years,20 to 29 years and 30 to 39 years was relatively high,the proportion of males and females in patients aged 40 to 49 years was similar,and the proportion of females in patients aged 50 to 59 years,60 to 69 years and 70 to 79 years was higher than that of males,and the proportion of males in patients aged>79 years was higher.Among the 70893 prescriptions selected in this study,alprazolam tablet was the most used,accounting for 36.29%;Followed by Zopiclone tablet,Clonazepam tablet and Esazolam tablet,accounting for 15.93%,15.28%and 10.02%,respectively.Among the 70,893 prescriptions selected in this study,1315 were unreasonable prescriptions,accounting for 1.90%.Irrational prescriptions mainly included inappropriate combination of drug use,inappropriate selection of drugs,inappropriate usage and dosage,inappropriate duration of drug use,inappropriate timing of drug administration,and off-label drug use,accounting for 30.27%,23.35%,22.21%,11.63%,8.37%,4.18%,respectively.Conclusion:The study showed that Alprazolam tablets,Zopiclone tablets,Clonazepam tablets and Esazolam tablets were commonly used in the treatment of patients with depression and sleep disorders in this hospital.Most of the drugs were reasonable in use,while a few were unreasonable,and medication guidance should be strengthened.
