摘　　要：目的系统整合中国MSM发生无保护性肛交影响因素的质性研究,为实施有效干预,减少艾滋病传播提供循证依据。方法计算机检索PubMed、Web of Science、CINHAL、Embase、Cochrane Library、中国知网、万方数据库等数据库中关于中国MSM发生无保护性肛交影响因素的质性研究,检索时限为建库至2024年7月,根据JBI循证卫生保健中心质性研究质量评价标准对所筛选的文献进行质量评价,使用汇集性整合法进行结果整合。结果共纳入13篇文献,提取出45个研究结果,归纳为12个类别,对中国MSM发生无保护性肛交的影响因素可以整合为5个结果:MSM对安全套的态度和认知、MSM对艾滋病的态度和认知、MSM发生无保护性肛交的个体因素、MSM发生无保护性肛交的群体因素、MSM在无保护性肛交前的物质和药物使用。结论中国MSM发生无保护性肛交的行为受多方面因素影响,应采取综合及个性化措施对其进行干预,提高安全套的使用率,普及相关知识,减少HIV的传播。Objective To systematically integrate qualitative studies on factors influencing unprotected anal intercourse among MSM in China and provide evidence for effective interventions to reduce HIV transmission.Methods A computer-based search was conducted in such databases as PubMed,Web of Science,CINHAL,Embase,Cochrane Library,CNKI,and Wanfang for qualitative studies on the influencing factors of unprotected anal intercourse among MSM in China(from database inception to July 2024).Literature quality was evaluated using the JBI Critical Appraisal Checklist for Qualitative Research,and the results were integrated using a meta-synthesis approach.Results Thirteen studies were included,and 45 findings were extracted.Findings were divided into 12 categories and integrated into five main results:attitudes and perceptions of MSM toward condoms,knowledge and attitudes of MSM toward HIV/AIDS,individual and group factors influencing unprotected anal intercourse,group factors influencing unprotected anal intercourse,and substance and drug use before unprotected anal intercourse.Conclusions Unprotected anal intercourse among MSM is influenced by several factors.Comprehensive and personalized interventions are required to increase condom use,disseminate relevant knowledge,and reduce HIV transmission.
