重症肺部感染患者应用乙酰半胱氨酸雾化联合美罗培南的效果分析

摘　　要：目的分析重症肺部感染患者应用乙酰半胱氨酸(富露施)雾化联合美罗培南治疗的效果。方法选取2021年6月至2023年6月收治的78例重症肺部感染患者,按随机数字表法分为对照组和研究组,每组39例。入组者均给予对症治疗,在此基础上对照组39例给予美罗培南治疗,研究组在对照组基础上联合富露施雾化治疗,对比2组临床疗效、恢复时间、动脉血气分析及炎性因子。结果研究组总有效率为94.87%,高于对照组76.92%(P<0.05);研究组痰菌转阴、肺啰音消失、体温恢复及咳痰消失时间较对照组更短(P<0.05);治疗后研究组SaO_(2)、TGF-β及PaO_(2)水平高于对照组,PCT、ACTH、PaCO_(2)及IL-1β水平较对照组更低(P<0.05)。结论重症肺部感染患者应用富露施雾化联合美罗培南治疗,可改善动脉血气分析指标,减轻患者炎症反应,并缩短恢复时间。Objective To analyze the effect of applying fulvestrant nebulization combined with meropenem on treating severe lung infections.Methods Seventy-eight patients with severe lung infections admitted from June 2021 to June 2023 were selected,and randomly assigned into the control group(n=39,symptomatic treatment plus meropenem)and study group(n=39,symptomatic treatment plus meropenem and fulvestrant nebulization).Clinical efficacy,recovery time,arterial blood gas indicators,and inflammatory factors of the two groups were compared.Results The total effective rate of the study group was significantly higher than that of the control group(94.87%vs 76.92%,P<0.05).The time for sputum negativity conversion,disappearance of pulmonary rales,recovery of body temperature and disappearance of sputum was significantly shorter in the study group than that of the control group(P<0.05).Arterial oxygen saturation(SaO_(2)),transforming growth factor-beta(TGF-β)and partial pressure of oxygen(PaO_(2))in the study group were significantly higher than those of the control group after treatment,and the levels of procalcitonin(PCT),adrenocorticotropic hormone(ACTH),partial pressure of carbon dioxide(PaCO2)and interleukin-1beta(IL-1β)were significantly lower than that of the control group(P<0.05).Conclusion The application of fulvestrant nebulization combined with meropenem in the treatment of patients with severe pulmonary infections can improve the indexes of arterial blood gas,reduce the inflammatory response,and shorten the recovery time.
