加味桂枝茯苓丸联合米非司酮治疗气滞血瘀型子宫肌瘤的临床效果

摘　　要：目的探讨气滞血瘀型子宫肌瘤患者应用加味桂枝茯苓丸联合米非司酮治疗的临床效果。方法选取2021年6月至2022年6月江西省中西医结合医院收治的80例子宫肌瘤患者作为研究对象,采用随机数字表法将其分为对照组(40例)和观察组(40例)。对照组采用米非司酮治疗,观察组在对照组方法的基础上,加用加味桂枝茯苓丸治疗,两组均连续治疗12周。比较两组的临床疗效、激素水平、最大肌瘤体积、肌瘤数量、中医证候积分、血液流变学指标、白细胞介素-6(IL-6)水平、血管内皮生长因子(VEGF)水平及不良反应。结果观察组的临床疗效优于对照组,总有效率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗前的激素水平、最大肌瘤体积、肌瘤数量、中医证候积分、血液流变学指标、IL-6、VEGF水平比较,差异无统计学意义(P>0.05);两组患者治疗后的激素水平、最大肌瘤体积、肌瘤数量、中医证候积分、血液流变学指标、IL-6、VEGF水平均低于本组治疗前,且观察组治疗后的以上各项指标均低于对照组,差异有统计学意义(P<0.05)。两组均未见明显不良反应。结论在米非司酮治疗基础上,加用加味桂枝茯苓丸治疗可显著提高治疗效果,对改善激素水平、抑制子宫肌瘤生长、缩小子宫肌瘤体积、改善血液流变学指标、减轻炎症反应、抑制VEGF表达方面效果较佳,且安全性好,值得推广。Objective To investigate the clinical effect of supplemented Guizhi Fuling Pills combined with Mifepristone in the treatment of uterine fibroids of qi stagnation and blood stasis type.Methods A total of 80 patients with uterine fibroids admitted to Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from June 2021 to June 2022 were selected as the research objects,and they were divided into control group(40 cases)and observation group(40 cases)by random number table method.The control group was treated with Mifepristone,and the observation group was treated with supplemented Guizhi Fuling Pills on the basis of the control group.Both groups were treated for 12 weeks.The clinical efficacy,hormone levels,maximum fibroid volume,fibroid number,traditional Chinese medicine syndrome score,hemorheology indexes,interleukin-6(IL-6)level,vascular endothelial growth factor(VEGF)level and adverse reactions were compared between the two groups.Results The clinical efficacy of the observation group was better than that of the control group,and the total effective rate was higher than that of the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in hormone levels,maximum fibroid volume,fibroid number,traditional Chinese medicine syndrome score,hemorheology indexes,IL-6 and VEGF levels between the two groups before treatment(P>0.05).After treatment,the hormone levels,maximum fibroid volume,fibroid number,traditional Chinese medicine syn-drome scores,hemorheology indexes,IL-6 and VEGF levels of the two groups were lower than those before treatment,and the above indexes in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no obvious adverse reactions in the two groups.Conclusion On the basis of Mifepristone treatment,supplemented Guizhi Fuling Pills can significantly improve the therapeutic effect,improve hormone levels,inhibit the growth of uterine fibro
