摘  要:目的观察糖尿病性骨质疏松症患者中抑瘤素M(OSM)水平变化,分析其与骨代谢、骨转换指标的关系,探讨OSM在糖尿病性骨质疏松症发生发展中的作用。方法选取2021年7月至2023年7月收治的2型糖尿病(T2DM)患者113例,根据骨质疏松诊断标准分为正常骨量组28例、骨量减少组39例和骨质疏松组46例。收集患者一般资料,测定生化指标、骨代谢、骨转换指标及血清OSM水平。分析血清OSM水平与骨密度(BMD)、骨代谢、骨转换指标的相关性,Logistic回归分析糖尿病性骨质疏松症的影响因素。结果3组体重指数、糖化血红蛋白(HbA1c)、25羟维生素D3[25(OH)D3]、甲状旁腺激素(PTH)、骨钙素、1型前胶原N端肽(PINP)、β-I型胶原交联羧基末端肽(β-CTX)、骨密度(BMD)比较,差异均有统计学意义(P<0.05);骨质疏松组、骨量减少组OSM水平低于正常骨量组,且骨质疏松组OSM水平低于骨量减少组,差异有统计学意义(P<0.05);血清OSM水平与25(OH)D3、骨钙素、PINP、BMD呈正相关,与PTH、β-CTX呈负相关(P<0.05);Logistic回归分析结果显示,体重指数、PINP、OSM、β-CTX是糖尿病性骨质疏松症的影响因素(P<0.05)。结论OSM水平变化与糖尿病性骨质疏松症发生密切相关,OSM水平是T2DM患者并发骨质疏松的保护因素。Objective To observe the expression changes of oncostatin M(OSM)in patients with diabetic osteoporosis,and to analyze its relationship with bone metabolism and bone turnover parameters.Moreover,to explore the role of OSM in the occurrence and development of diabetic osteoporosis.Methods A total of 113 patients with type 2 diabetes mellitus(T2DM)admitted to our hospital from July 2021 to July 2023 were selected.According to the diagnostic criteria of osteoporosis,they were divided into normal bone mass group(28 cases),osteopenia group(39 cases)and osteoporosis group(46 cases).General data of patients were collected.Biochemical,bone metabolism and bone turnover parameters,as well as serum OSM levels were determined.The correlation of serum OSM level with bone mineral density(BMD),bone metabolism and bone turnover was analyzed.The influencing factors of diabetic osteoporosis were analyzed by Logistic regression.Results There were significant differences in body mass index(BMI),glycosylated hemoglobin(HbA1c),25-hydroxyvitamin D3(25(OH)D3),parathyroid hormone(PTH),osteocalcin,N-terminal propeptide of type I procollagen(PINP),β-carboxy-terminal cross-linking telopeptide of type I collagen(β-CTX)and bone mineral density(BMD)among the three groups(P<0.05).The OSM level of osteoporosis group and osteopenia group was significantly lower than that of normal bone mass group,and it was significantly lower in osteoporosis group than that of osteopenia group(P<0.05).Serum OSM level was positively correlated with 25(OH)D3,osteocalcin,PINP and BMD,but negatively correlated with PTH andβ-CTX(P<0.05).Logistic regression analysis showed that BMI,PINP,OSM andβ-CTX were the influencing factors of diabetic osteoporosis(P<0.05).Conclusion The change of OSM level is closely related to the occurrence of diabetic osteoporosis,and OSM level is a protective factor for osteoporosis in T2DM patients.
