三角韧带加强修补联合切开复位内固定治疗踝关节骨折合并三角韧带损伤的临床效果

摘　　要：目的探讨踝关节骨折(ankle fracture,AF)合并三角韧带(deltoid ligament,DL)损伤采用DL加强修补联合切开复位内固定(open reduction and internal fixation,ORIF)治疗的临床效果。方法随机选取2021年1月—2024年2月常州市新北区三井人民医院收治的60例AF合并DL损伤患者作为研究对象,以不同治疗方案分为对照组(n=30)和研究组(n=30)。对照组给予ORIF治疗,研究组给予DL加强修补联合ORIF治疗,比较两组踝关节恢复效果、手术相关指标、复位情况、踝-后足功能、并发症发生情况。结果研究组踝关节恢复总优良率为96.67%(29/30),高于对照组的73.33%(22/30),差异有统计学意义(χ^(2)=4.706,P<0.05)。研究组手术时间长于对照组,住院时间和骨折愈合时间均短于对照组,差异有统计学意义(P均<0.05)。术后1个月,两组内踝间隙、距骨倾斜角均下降,研究组低于对照组,差异有统计学意义(P均<0.05)。术后1个月,两组踝-后足功能评分均升高,研究组高于对照组,差异有统计学意义(P均<0.05)。研究组并发症总发生率为0,低于对照组的20.00%(6/30),差异有统计学意义(χ^(2)=4.630,P<0.05)。结论DL加强修补联合ORIF应用于AF合并DL损伤治疗中,虽手术时间较长,但能够改善踝关节恢复效果,加快骨折愈合,降低内踝间隙、距骨倾斜角,提高踝-后足功能,减少并发症发生。Objective To investigate the clinical effect of ankle fracture(AF)combined with deltoid ligament(DL)in⁃jury treated with DL reinforcement repair combined with open reduction and internal fixation(ORIF).Methods From January 2021 to February 2024,60 patients with AF combined with DL injury admitted to Changzhou Xinbei District Sanjing People's Hospital were randomly selected as the research objects.They were divided into control group(n=30)and study group(n=30)according to different treatment schemes.The control group was treated with ORIF,and the study group was treated with DL enhanced repair combined with ORIF.The ankle joint recovery effect,operation-related indicators,reduction,ankle-hind foot function and complications were compared between the two groups.Re⁃sults The total excellent and good rate of ankle joint the study group was 96.67%(29/30),which was higher than 73.33%(22/30)of the control group,and the difference was statistically significant(χ^(2)=4.706,P<0.05).The operation time of the study group was longer than that of the control group,and the hospitalization time and fracture healing time were all shorter than those of the control group,the differences were statistically significant(all P<0.05).One month after operation,the medial malleolus space and talus inclination angle of the two groups decreased,and those of the study group were lower than the control group,the differences were statistically significant(all P<0.05).One month af⁃ter operation,the ankle-hind foot function scores of the two groups increased,and the study group was higher than the control group,the differences were statistically significant(both P<0.05).The total incidence of complications in the study group was 0,which was lower than 20.00%(6/30)in the control group,and the difference was statistically sig⁃nificant(χ^(2)=4.630,P<0.05).Conclusion DL reinforcement repair combined with ORIF is applied to the treatment of AF combined with DL injury.Although the operation time is long,it can improve the recovery effect of
