经尿道前列腺钬激光剜除术治疗大体积前列腺增生老年患者的有效性及安全性分析

摘　　要：目的探讨经尿道前列腺钬激光剜除术在>80 mL大体积前列腺增生患者中的应用。方法回顾性选取2021年7月—2023年7月贵航贵阳医院收治的120例>80 mL大体积前列腺增生老年患者的临床资料,按照不同手术方法分为对照组和研究组,行经尿道前列腺电切术患者为对照组,行经尿道前列腺钬激光剜除术患者为研究组,每组60例。对比两组围手术期指标、术后恢复情况、并发症发生情况。结果术后6个月,研究组残余尿量、国际前列腺症状评分、生活质量评分均低于对照组,最大尿流率高于对照组,差异有统计学意义(P均<0.05)。研究组的手术时间、住院时长和留置导管时长均短于对照组,术中出血量少于对照组,差异有统计学意义(P均<0.05)。研究组术后并发症发生率为3.33%(2/60),低于对照组的15.00%(9/60),差异有统计学意义(χ^(2)=4.904,P<0.05)。结论采用经尿道前列腺钬激光剜除术治疗>80 mL大体积的前列腺增生老年患者,可改善患者术后恢复指标及围手术期相关情况,缩短手术时间,且安全性较高。Objective To investigate the application of transurethral holmium laser enucleation of the prostate in pa⁃tients with large volume prostatic hyperplasia>80 mL.Methods The clinical data of 120 elderly patients with large-volume prostatic hyperplasia>80 mL admitted to Guihang Guiyang Hospital from July 2021 to July 2023 were retro⁃spectively selected and divided into control group and study group according to different surgical methods.The pa⁃tients undergoing transurethral resection of the prostate were the control group,and the patients undergoing transure⁃thral holmium laser enucleation of the prostate were the study group,with 60 cases in each group.The perioperative indexes,postoperative recovery and complications were compared between the two groups.Results At 6 months after operation,the residual urine volume,International Prostate Symptom Score and quality of life score of the study group were lower than those of the control group,and the maximum urinary flow rate was higher than that of the control group,the differences were statistically significant(all P<0.05).The operation time,hospitalization time and indwell⁃ing catheter time of the study group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(all P<0.05).The incidence of postop⁃erative complications in the study group was 3.33%(2/60),which was lower than 15.00%(9/60)in the control group,and the difference was statistically significant(χ^(2)=4.904,P<0.05).Conclusion Transurethral holmium laser enucle⁃ation of the prostate for the treatment of elderly patients with large volume of benign prostatic hyperplasia>80 mL can improve the postoperative recovery index and perioperative related conditions,shorten the operation time,and has high safety.
