通络活血方联合循经按摩治疗腰椎间盘突出症临床研究

摘　　要：目的:观察通络活血方联合循经按摩治疗腰椎间盘突出症的疗效。方法:选取104例腰椎间盘突出症患者,按随机数字表法分为联合组及对照组各52例。对照组接受循经按摩,联合组接受通络活血方联合循经按摩治疗。比较2组临床疗效,比较2组治疗前后中医症状积分、疼痛视觉模拟评分法(VAS)评分、日本骨科协会(JOA)评分、生活质量评估量表(SF-36)评分、下肢神经传导速度的变化。结果:联合组临床疗效总有效率为94.23%,对照组78.85%,2组临床疗效比较,差异有统计学意义(P<0.05)。治疗后,2组中医症状积分、VAS评分均较治疗前下降(P<0.05),联合组中医症状积分、VAS评分均低于对照组(P<0.05)。治疗后,2组JOA评分、SF-36评分均较治疗前升高(P<0.05),联合组JOA评分、SF-36评分均高于对照组(P<0.05)。治疗后,2组腓总神经传导速度、胫神经传导速度均较治疗前提升(P<0.05),联合组腓总神经传导速度、胫神经传导速度均快于对照组(P<0.05)。结论:通络活血方联合循经按摩可有效缓解腰椎间盘突出症腰部疼痛症状,改善腰椎功能和生活质量。Objective:To observe the efficacy of Tongluo Huoxue Prescription combined with meridian massage in the treatment of lumbar disc herniation.Methods:A total of 104 patients with lumbar disc herniation were selected and randomly divided into the combination group and the control group,with 52 cases in each group.The control group received meridian massage,and the combination group received meridian massage combined with Tongluo Huoxue Prescription.The clinical efficacy in the two groups was compared;the changes in traditional Chinese medicine symptom scores,Visual Analogue Scale(VAS)scores of pain,Japanese Orthopaedic Association(JOA)scores,36-item Short-Form(SF-36)scores,and lower limb nerve conduction velocity before and after treatment were compared between the two groups.Results:The total effective rate of clinical treatment was 94.23%in the combination group and 78.85%in the control group,the difference in clinical efficacy between the two groups being significant(P<0.05).After treatment,the traditional Chinese medicine symptom scores and VAS scores in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the combination group were lower than those in the control group(P<0.05).After treatment,the JOA scores and SF-36 scores in the two groups were increased when compared with those before treatment(P<0.05),and the scores in the combination group were higher than those in the control group(P<0.05).After treatment,the common peroneal nerve conduction velocity and tibial nerve conduction velocity in the two groups were improved when compared with those before treatment(P<0.05),and these velocities in the combination group were faster than those in the control group(P<0.05).Conclusion:Tongluo Huoxue Prescription combined with meridian massage can effectively relieve lumbar pain symptoms of lumbar disc herniation,and improve lumbar function and quality of life.
