摘　　要：目的探讨巴氯酚联合火针治疗卒中痉挛的疗效和机制。方法选取2022年8月—2023年8月滨州市第二人民医院收治的100例卒中痉挛患者作为研究对象,根据不同治疗方案分为两组,各50例。所有患者均接受常规康复治疗,对照组给予巴氯酚治疗,火针组在对照组基础上给予火针治疗,两组均观察8周,比较两组康复前后肘关节及膝关节痉挛评分、上肢及下肢肌力分级评分与上肢及下肢运动功能评分。结果两组康复治疗8周后的肘关节、膝关节痉挛评分均低于康复治疗前,且火针组肘关节、膝关节痉挛评分为(1.79±0.16)分、(1.72±0.22)分,均低于对照组的(2.47±0.16)分、(2.33±0.17)分,差异有统计学意义(t=21.250、15.514,P均<0.05)。康复治疗8周后,两组上肢、下肢肌力分级评分均高于康复治疗前,且火针组高于对照组,差异有统计学意义(P均<0.05)。康复8周后两组上肢、下肢Brunnstrom运动功能评分均高于康复治疗前,且火针组高于对照组,差异有统计学意义(P均<0.05)。结论巴氯酚联合火针治疗卒中痉挛能缓解痉挛症状,改善患者的上肢、下肢肌力与运动功能。Objective To explore the efficacy and mechanism of baclofen combined with fire needle in the treatment of stroke spasm.Methods A total of 100 patients with stroke spasm admitted to the Second People's Hospital of Binzhou from August 2022 to August 2023 were selected as the research objects.According to the different treatment method,they were divided into two groups,with 50 cases in each group.All patient received routine rehabilitation treatment.The control group was treated with baclofen,and the fire needle group was treated with fire needle on the basis of the control group.Both groups were observed for 8 weeks The spasm scores of elbow joint and knee joint,muscle strength scores of upper limb and lower limb and motor function scores of upper limb and lower limb before and after rehabilitation were compared between the two groups.Results The scores of elbow joint and knee joint spasm in the two groups after 8 weeks of rehabilitation treatment were lower than those before rehabilitation treatment,and the scores of elbow joint[(1.79±0.16)points and knee joint spasm(1.72±0.22)points]in the fire needle group were lower than those in the control group[(2.47±0.16)points and(2.33±0.17)points],and the differences were statistically significant(t=21.250,15.514,both P<0.05).After 8 weeks of rehabilitation treatment,the upper and lower limb muscle strength grading scores of the two groups were higher than those before rehabilitation treatment,and the fire needle group was higher than the control group,the differences were statistically significant(both P<0.05).After 8 weeks of rehabilitation treatment,the Brunnstrom motor function scores of the upper and lower limbs of the two groups were higher than those before rehabilitation treatment,and the fire needle group was higher than the control group,and the differences were statistically significant(both P<0.05).Conclusion Baclofen combined with fire needle in the treatment of stroke spasm can relieve spasm symptoms and improve the muscle strength and motor function of
