摘　　要：目的分析髓内钉内固定治疗肱骨近端骨折疗效和安全性。方法选取2020年1月—2024年3月毕节市第二人民医院收治的60例肱骨近端骨折患者为研究对象,按照手术方法不同分为对照组和观察组,各30例。观察组采用髓内钉内固术,对照组采用锁定钢板内固术。比较两组治疗效果、并发症发生情况、骨折愈合时间。结果治疗后,观察组手术时长、术内透视频次、骨折康复时间均少于对照组,Constant-Murley肩部功能评估标准评分高于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为10.00%(3/30),低于对照组的33.33%(10/30),差异有统计学意义(χ^(2)=4.812,P<0.05)。治疗后,观察组骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。结论针对肱骨近端骨折患者,采用股髓内钉内固定治疗效果较好,可提升关节功能和日常生活能力,降低并发症发生率,缩短骨折康复周期。Objective To analyze the efficacy and safety of intramedullary nail fixation in the treatment of proximal humeral fractures.Methods A total of 60 patients with proximal humeral fractures admitted to the Second People's Hospital of Bijie from January 2020 to March 2024 were selected as the research objects.According to different surgical methods,they were divided into the control group and the observation group,with 30 cases in each group.The observation group was treated with intramedullary nail fixation,and the control group was treated with locking plate fixation.The treatment effects,complications,and fracture healing time were compared between the two groups.Results After treatment,the operation time,frequency of intraoperative fluoroscopy,and fracture rehabilitation time of the observation group were less than those of the control group,and the Constant-Murley shoulder function evaluation standard score was higher than that of the control group,and the differences were statistically significant(all P<0.05).The total incidence of complications in the observation group was 10.00%(3/30),which was lower than 33.33%(10/30)in the control group,and the difference was statistically significant(χ^(2)=4.812,P<0.05).After treatment,the fracture healing time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with proximal humeral fractures,femoral intramedullary nail internal fixation has a good effect,which can improve joint function and daily living ability,reduce the incidence of complications,and shorten the fracture rehabilitation period.
