摘 要:目的探究多层螺旋CT(multi-slice CT,MSCT)三维重建辅助3D打印技术治疗高龄不稳定股骨粗隆间骨折的临床疗效。方法选取2022年7月—2023年6月梧州市红十字会医院收治的90例复杂股骨粗隆间骨折患者为研究对象,按术前计划的不同分为研究组(n=45)与对照组(n=45)。两组均行手术治疗,对照组以传统方法行术前计划,研究组以MSCT三维重建辅助3D打印技术行术前计划。比较两组手术指标、Harris髋关节评分和并发症总发生率。结果研究组手术时间、术中出血量、术中透视次数均少于对照组,差异有统计学意义(P均<0.05)。研究组一次置钉成功率为82.22%(37/45),高于对照组的62.22%(28/45),差异有统计学意义(χ^(2)=4.486,P=0.034)。研究组术后1、3、6个月Harris髋关节评分均高于对照组,差异有统计学意义(P均<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论MSCT三维重建辅助3D打印技术治疗高龄不稳定股骨粗隆间骨折可确保骨折精确复位固定,缩短患者住院时间,促进术后恢复,获得较好的治疗效果。Objective To explore the clinical efficacy of multi-slice CT(MSCT)three-dimensional reconstruction assisted 3D printing in the treatment of unstable intertrochanteric fractures in the elderly.Methods A total of 90 patients with complex femoral intertrochanteric fractures admitted to Wuzhou Red Cross Hospital from July 2022 to June 2023 were selected as the research objects and divided into study group(n=45)and control group(n=45)according to different preoperative planning.Both groups underwent surgical treatment.The control group underwent preoperative planning with traditional methods,and the study group underwent preoperative planning with MSCT three-dimensional reconstruction assisted 3D printing technology.The operation indexes,Harris Hip Scoring System score and total incidence of complications were compared between the two groups.Results The operation time,intraoperative blood loss and intraoperative fluoroscopy times in the study group were less than those in the control group,and the differences were statistically significant(all P<0.05).The success rate of one-time screw placement in the study group was 82.22%(37/45),which was higher than 62.22%(28/45)in the control group,and the difference was statistically significant(χ^(2)=4.486,P=0.034).The Harris Hip Scoring System scores of the study group were higher than those of the control group at 1,3 and 6 months after operation,and the differences were statistically significant(all P<0.05).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion MSCT three-dimensional reconstruction assisted 3D printing technology in the treatment of unstable intertrochanteric fractures in the elderly can ensure accurate reduction and fixation of fractures,shorten the hospitalization time of patients,promote postoperative recovery,and obtain better therapeutic effect.
