摘 要:目的探讨磁共振成像(magnetic resonance imaging,MRI)与多层螺旋CT在膝关节损伤诊断中的效果。方法选择2020年1月—2023年10月丹阳市第三人民医院收治的76例疑似膝关节损伤患者为研究对象,均开展MRI与多层螺旋CT检查,以关节镜检查结果为金标准,评估MRI与多层螺旋CT的诊断效能以及对不同类型损伤的诊断符合率。结果经关节镜检查,76例患者确诊阳性59例、阴性17例。MRI对膝关节损伤诊断的准确度为97.37%(74/76)、灵敏度为98.30%(58/59),均高于多层螺旋CT的84.21%(64/76)、84.74%(50/59),差异有统计学意义(χ^(2)=10.000、6.125,P均<0.05)。两种检查方法的特异度比较,差异无统计学意义(P>0.05)。两种检查方法对关节腔积液、骨折诊断符合率比较,差异无统计学意义(P均>0.05)。MRI对半月板损伤及韧带损伤诊断符合率高于多层螺旋CT%,差异有统计学意义(P均<0.05)。结论MRI检查对膝关节损伤的诊断效能高于多层螺旋CT,且在半月板损伤及韧带损伤的诊断上,MRI更具有优势。Objective To investigate the effect between magnetic resonance imaging(MRI)and multi-slice Spiral CT(MSCT)in the diagnosis of knee joint injury.Methods A total of 76 patients with suspected knee joint injury were selected from the Third People's Hospital of Danyang City from January 2020 to October 2023 as the study objects,and MRI and MSCT examination were performed.The difference in diagnostic efficiency and the diagnostic coincidence rate for different types of injury between MRI and MSCT were compared with the results of arthroscopy as the gold standard.Results After arthroscopic examination,59 cases were positive and 17 cases were negative in 76 patients with suspected knee joint injury.The accuracy and sensitivity of MRI in the diagnosis of knee joint injury were 97.37%(74/76)and 98.30%(58/59),which were higher than 84.21%(64/76)and 84.74%(50/59)those of MSCT,and the differences were statistically significant(χ^(2)=10.000,6.125,both P<0.05).There was no significant difference in the specificity between the two methods(P>0.05).There was no significant difference in the diagnostic coincidence rate of joint effusion and fracture between the two methods(both P>0.05).The diagnostic coincidence rates of meniscus injury and ligament injury by MRI were higher than MSCT,and the differences were statistically significant(both P<0.05).Conclusion MRI is more effective than MSCT in the diagnosis of knee injuries,and MRI has more advantages in meniscus injury and ligament.
