外周白细胞、C反应蛋白和血清降钙素原检测在肺结核合并肺部细菌感染中的诊断价值

摘　　要：目的探讨外周白细胞(white blood cell,WBC)、C反应蛋白(C-reactive protein,CRP)和血清降钙素原(procalcitonin,PCT)三种血清指标运用于肺结核伴肺部细菌感染患者中的诊断效能。方法目的选取2019年3月—2022年1月高州市人民医院收治的80例患者肺结核患者为研究对象,其中40例患者为肺结核合并肺部细菌感染者,设为观察组,40例单纯肺结核患者设为对照组。对比两组患者PCT、CRP以及WBC水平的差异、对比肺部细菌感染不同严重程度的患者PCT、CRP以及WBC指标水平的差异,以及分析上述检测指标单一和联合对肺结核伴肺部细菌感染患者的诊断价值。结果观察组患者的WBC、CRP、PCT水平均高于对照组,差异有统计学意义(P均<0.05)。不同严重程度患者的PCT、CRP及WBC水平对比,差异有统计学意义(P均<0.05)。WBC、PCT及CRP联合诊断的AUC为0.924(95%CI:0.868~0.981)。WBC、PCT、CRP三者联合诊断肺结核合并肺部细菌感染的灵敏度与特异度高于各单一指标诊断效能。结论WBC、CRP和PCT在肺结核合并肺部细菌感染的患者中表达水平较高,三个指标联合用于辅助诊断肺结核合并肺部细菌感染的价值较高。Objective To investigate the efficacy of application of peripheral white blood cell(WBC),C-reactive protein(CRP)and serum procalcitonin(PCT)in the diagnosis of pulmonary tuberculosis with pulmonary bacterial infection.Methods Eighty patients with pulmonary tuberculosis admitted to Gaozhou People's Hospital from March 2019 to January 2022 were purposefully selected as the research objects.Among them,forty patients with pulmonary tuberculosis complicated with pulmonary bacterial infection were set as the observation group,and forty patients with simple pulmonary tuberculosis were set as the control group.The differences of PCT,CRP and WBC levels between the two groups were compared,and the differences of PCT,CRP and WBC levels in patients with different severity of pulmonary bacterial infection were compared,and the diagnostic value of single and combined detection of the above indicators in patients with pulmonary tuberculosis complicated with pulmonary bacterial infection was analyzed.Results The levels of WBC,CRP and PCT in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The levels of PCT,CRP and WBC in patients with different severity were compared,and the differences were statistically significant(all P<0.05).The AUC of combined diagnosis of WBC,PCT and CRP was 0.924(95%CI:0.868~0.981).The sensitivity and specificity of the combination of WBC,PCT and CRP in the diagnosis of pulmonary tuberculosis complicated with pulmonary bacterial infection were higher than each single index diagnosis.Conclusion The expression levels of WBC,CRP and PCT are high in patients with pulmonary tuberculosis complicated with pulmonary bacterial infection.The combination of the three indicators is of high value in the diagnosis of pulmonary tuberculosis complicated with pulmonary bacterial infection.
