轻比重罗哌卡因复合芬太尼腰麻在老年髋部手术麻醉中的应用分析

摘　　要：目的探究对老年髋部手术患者采取轻比重罗哌卡因复合芬太尼在腰麻中的效果。方法非随机选取泗阳康达医院于2022年3月—2023年6月收治的62例老年髋部手术患者为研究对象,依据麻醉方案不同分为对照组和观察组,每组31例。对照组实施10%葡萄糖注射液+布比卡因,观察组实施轻比重罗哌卡因复合芬太尼,比较两组麻醉各项指标、心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)。结果两组运动阻滞起效时间、感觉阻滞起效时间对比,差异无统计学意义(P均>0.05)。与T0时比,T1~T3时两组患者HR、MAP均先降低后升高,并且麻醉后在T1~T3观察组HR以及MAP波动幅度均较小,差异有统计学意义(P均<0.05)。结论在对老年髋部手术患者麻醉时,给予轻比重罗哌卡因复合芬太尼在腰麻中,效果较好,对循环系统影响较小。Objective To explore the effect of compound fentanyl in elderly patients with hip surgery.Methods Sixty-two elderly hip surgery patients admitted to Siyang Kangda Hospital from March 2022 to June 2023 were non-randomly selected as the study objects,and were divided into control group and observation group according to different anesthesia protocols,with thirty-one cases in each group.The control group received 10%glucose injection plus bupivacaine,and the observation group received light weight ropivacaine combined with fentanyl.The anesthesia indexes,heart rate(HR)and mean arterial pressure(MAP)of the two groups were compared.Results There was no statistically significant difference in the onset time of motor block and sensory block between the two groups(both P>0.05).Compared with T0,HR and MAP in the two groups decreased first and then increased at T1-T3,and the fluctuation amplitude of HR and MAP in the T1-T3 observation group after anesthesia was small,the differences were statistically significant(all P<0.05).Conclusion In the anesthesia of elderly patients with hip surgery,the effect of light weight ropivacaine combined with fentanyl in lumbar anesthesia is better,and the circulatory system is less affected.
