关节镜微骨折手术与自体骨软骨移植手术治疗距骨骨软骨损伤的临床疗效分析

摘  要:目的分析关节镜微骨折手术、自体骨软骨移植手术治疗距骨骨软骨损伤的效果。方法非随机选取2021年1月—2023年10月新沂市人民医院收治的78例距骨骨软骨损伤患者为研究对象,按手术方式分为两组,每组39例,对照组行关节镜微骨折手术,观察组行自体骨软骨移植手术,对比两组手术指标、治疗有效率及恢复情况。结果观察组切口长度较对照组长,术中出血量较对照组多,手术时间、住院时间较对照组长,差异有统计学意义(P均<0.05)。观察组的总有效率为97.44%(38/39),高于对照组的79.49%(31/39),差异有统计学意义(χ^(2)=4.522,P<0.05)。观察组踝关节功能评分、踝关节活动度高于对照组,差异有统计学意义(P均<0.05)。结论自体骨软骨移植手术同关节镜微骨折手术均可治疗距骨骨软骨损伤,后者虽然手术耗时较长、出血量较多,但其治疗有效率高,术后恢复更理想。Objective To analyze the effects of arthroscopic microfracture and autogenous osteochondral transplantation in the treatment of osteochondral injury of talus.Methods A total of seventy-eight patients with osteochondral injury of talus treated in Xinyi People's Hospital from January 2021 to October 2023 were non-randomly selected as the study objects.They were divided into two groups according to the surgical method,with thirty-nine cases in each group.The control group received arthroscopic microfracture surgery,and the observation group received autogenous osteochondral transplantation.Results The incision length of the observation group was longer than that of the control group,the amount of intraoperative blood loss was more,the operation time and hospital stay were longer than that of the control group,and the differences were statistically significant(all P<0.05).The total effective rate of the observation group was 97.44%(38/39),which was higher than 79.49%(31/39)of the control group,and the difference was statistically significant(χ^(2)=4.522,P<0.05).The ankle joint function score and ankle joint motion in the observation group were higher than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion Both autogenous osteochondral transplantation and arthroscopic microfracture can be used in the treatment of osteochondral injury of talus.Although the latter surgery takes longer time and involves more blood loss,it is highly effective and has better postoperative recovery.
