血必净注射液联合bFGF治疗烧伤创面的临床疗效观察

摘　　要：目的:探究血必净注射液联合碱性成纤维细胞生长因子(Basic fibroblast growth factor,bFGF)治疗Ⅱ、Ⅲ度烧伤患者的临床疗效。方法:选取2020年2月-2023年2月笔者医院烧伤科收治的102例Ⅱ、Ⅲ度烧伤患者作为研究对象,按照随机数字表法分为联合组与bFGF组,各51例。两组均给予常规烧伤处理,bFGF组加予bFGF喷剂治疗,联合组加予血必净注射液联合bFGF治疗。比较两组创面疼痛情况[视觉模拟评分法(Visual analogue scale,VAS)]、免疫炎症指标[细胞间黏附分子-1(Intercellular adhesion molecule-1,ICAM-1)、C反应蛋白(C reactive protein,CRP)、白细胞计数(White blood cell count,WBC)、降钙原素(Pocalcitonin,PCT)]、创面愈合率、创面愈合时间、临床疗效、不良反应发生情况及瘢痕形成情况[温哥华瘢痕量表(Vancouver scar scale,VSS)]。结果:治疗6、24 h,联合组VAS评分低于bFGF组(P<0.05);治疗7 d后,联合组外周血ICAM-1、CRP、WBC及PCT水平显著低于bFGF组(P<0.05);治疗14 d后,联合组创面愈合率和治疗总有效率高于bFGF组(P<0.05);联合组创面愈合时间短于bFGF组(P<0.05);两组不良反应发生情况比较差异无统计学意义(P>0.05)。治疗30 d后,联合组VSS各评分(色泽、柔软度、血管分布、厚度)均低于bFGF组(P<0.05)。结论:血必净联合bFGF的用药方案能够有效促进Ⅱ、Ⅲ度烧伤患者创面修复,具有明显止痛功效,药物无严重不良反应,且能减少瘢痕的发生,值得临床应用推广。Objective To explore the efficacy of Xuebijing injection combined with basic fibroblast growth factor(bFGF)in the treatment of patients with second-degree and third-degree burns and its influence on wound healing and pain score.Methods 102 patients with second-degree and third-degree burns who were received in department of burn of the author's hospital from February 2020 to February 2023 were selected as the study subjects.According to the random number table method,all patients were randomly divided into combined group and bFGF group with 51 cases in each group.All patients were given conventional burn treatment,and the bFGF group was treated with bFGF spray,and the combined group was given Xuebijing injection combined with bFGF.Wound pain[visual analogue scale(VAS)],the immune-inflammatory markers[intercellular adhesion molecule-1(ICAM-1),C reactive protein(CRP),White blood cell]were detected count(WBC),pocalcitonin(PCT),wound healing rate,wound healing time,clinical efficacy,adverse reactions and scar formation[Vancouver scar scale(VSS)]were compared between the two groups.Results At 6 and 24 hours after treatment,VAS scores in the combined group were lower than those in the bFGF group(P<0.05).After 7 days of treatment,the levels of ICAM-1,CRP,WBC and PCT in peripheral blood of the combined group were significantly lower than those of the bFGF group(P<0.05).After 14 days of treatment,the wound healing rate and total effective rate of treatment in combination group were higher than those in bFGF group(P<0.05).The wound healing time of the combined group was shorter than that of the bFGF group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 30 days of treatment,the VSS scores(color,softness,vascular distribution,and thickness)in the combined group were lower than those in the bFGF group(P<0.05).Conclusion The medication regimen of Xuebijing combined with bFGF can effectively promote the wound repair of patients with partial-thickness and ful
