关节病型银屑病并发髋周多处异位骨化1例

摘　　要：患者男,41岁,红色斑丘疹伴鳞屑17年,髋关节肿痛2个月,加重1个月。患者17年前无明显诱因出现头部、四肢、腹部皮肤红色斑丘疹伴鳞屑,外院诊断为“银屑病”;2个月前患者双侧髋关节疼痛,左侧为著,近1个月加重伴活动受限。此次入院查体左侧髋关节外侧可扪及肿块;髋关节CT示:双侧股骨上端、左侧髋关节周围见软组织肿块及骨性斑块;骶髂关节CT示:两侧骶髂关节炎改变。患者拒绝行髋关节处异位骨化病理活检。诊断:关节病型银屑病并发髋周多处异位骨化。A 41-year-old male presented with red maculopapules and scales that had been persisting for 17 years.He had also hip joint swelling and pain that had lasted for 2 months and worsened over the past month.The patient was diagnosed with psoriasis 17 years ago,which initially presented with red maculopapules and scales on the scalp,limbs,and abdomen without apparent triggers.Two months ago,the patient developed bilateral hip pain,predominantly on the left side,which progressively worsened over the past month,along with limited mobility.A palpable mass was observed on the lateral aspect of the left hip.CT imaging of the hip joints revealed a soft tissue mass and bony plaque surrounding the upper femur and left hip joints.Additionally,a CT scan of the sacroiliac joint demonstrated changes indicative of bilateral sacroiliac arthritis.Pathological biopsy to confirm ectopic ossification in the hip joint region was recommended,but it was declined by the patient.The final diagnosis was psoriatic arthritis complicated with multiple ectopic ossification of hip joints.
