止喘颗粒联合喘可治穴位注射治疗慢性阻塞性肺疾病急性加重期疗效观察

摘　　要：目的:观察止喘颗粒联合喘可治穴位注射治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)的临床疗效。方法:将120例AECOPD患者按随机数字表法分为对照组和观察组各60例。对照组予常规西医干预措施,观察组在此基础上予止喘颗粒联合喘可治穴位注射;连续治疗10天后比较两组患者治疗前后中医证候积分,肺功能指标[第一秒用力呼气量(forced expiratory volume in one second,FEV_(1)),第一秒用力肺活量/潮气量比值(first second forced vital capacity/tidal volume,FEV_(1)/FVC)],气道黏液分泌指标中性粒细胞弹力蛋白酶(neutrophil elastase,NE)、杯状细胞分泌黏液蛋白(mucin 5 subtype AC,MUC5AC)及炎性因子[白细胞介素4(interleukin-4,IL-4)、IL-17、干扰素γ(interferon-γ,IFN-γ)]水平与临床疗效。结果:观察组总有效率为93.3%(56/60),高于对照组的80.0%(48/60)(P<0.05);观察组患者中医证候积分低于对照组(P<0.05);FEV_(1)、FEV_(1)/FVC水平高于对照组(P<0.05)两组患者NE、MUC5AC水平均降低(P<0.05),观察组低于对照组(P<0.05);观察组IL-4、IL-17水平低于对照组,IFN-γ水平高于对照组(P<0.05)。结论:止喘颗粒联合喘可治穴位注射能够改善AECOPD患者临床症状和肺功能,减轻气道黏液高分泌状态,抑制炎性反应,效果优于常规西医干预措施。Objective:To observe therapeutic effects of asthma-relieving granules combined with acupoint injection of Chuankezhi(Asthma-healing Injection)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:A total of 120 AECOPD patients were divided into the control group and the observation group according to random number table method with 60 cases in each group.Conventional intervention measures of Western medicine was administered to the control group,while the observation group was given asthma-relieving granules and acupoint injection of Chuankezhi based on the treatment of the control group.To compare TCM syndrome integrals,the indexes of lung function including FEV_(1),FEV_(1)/FVC,airway mucus secretion indexes such as NE and MUC5AC and the levels of inflammatory factors including IL-4,IL-17 and IFN-γ,as well as clinical effecs before and after the treatment after ten days of treatment.Results:Total effective rate of the observation group was 93.3%(56/60),higher than 80.0%(48/60)of the control group(P<0.05);TCM syndrome integrals of the observation group were lower than these of the control group(P<0.05),the levels of FEV_(1)and FEV_(1)/FVC higher than these of the control group(P<0.05);the levels of NE and MUC5AC were decreased in the two groups(P<0.05),the observation group was lower than the control group(P<0.05);the observa-tion group was lower than the control group in the expressions of IL-4 and IL-17,higher than the control group in the levels of IFN-γ(P<0.05).Conclusion:Asthma-relieving granules combined with acupoint injection of Chuankezhi could improve AECOPD patients'clinical symptoms and lung function,alleviate airway mucus hypersecretion,inhibit inflammatory reaction,and its effects superior to conventional intervention measures of Western medicine.
