自拟中药组方联合康柏西普治疗糖尿病性黄斑水肿临床观察

摘　　要：目的观察自拟中药组方联合康柏西普治疗糖尿病性黄斑水肿(DME)的临床疗效。方法选取2023年6月-2023年9月在辽源市中医院眼科门诊就诊的DME患者(78例96眼),随机分为2组,对照组(39例,46眼)采用眼内注射康柏西普,观察组(39例,50眼)在对照组的基础上口服自拟中药组方,治疗周期为2个月,随访4个月。比较2组的治疗效果、并发症的发生率、治疗前后的最佳矫正视力及黄斑中心视网膜厚度以及治疗后4个月内因DME再次治疗的次数。结果观察组的治疗效果优于对照组(P<0.05)。2组并发症总发生率比较,差异无统计学意义(P>0.05)。治疗后2组的最佳矫正视力均有明显提高,且观察组优于对照组(P<0.001);2组的黄斑中心视网膜厚度均有明显降低,且观察组优于对照组(P<0.001)。治疗后4个月内,观察组因DME再次治疗的次数少与对照组(P<0.05)。结论自拟中药组方联合康柏西普治疗DME的疗效更为显著,可有效提高患者视力,降低黄斑水肿程度,减少眼内注药次数,降低复发率,具有临床应用价值。Objective To observe the clinical efficacy of self-made traditional Chinese compound combined with conbercept in the treatment of diabetic macular edema(DME).Methods 78 patients(96 eyes)with DME who visited ophthalmology outpatient department of Liaoyuan Hospital of Traditional Chinese Medicine from June 2023 to September 2023 were randomly divided into the control group and the observation group.The control group(39 cases,46 eyes)received intraocular injection of conbercept,and the observation group(39 cases,50 eyes)received oral administration of self-made traditional Chinese compound on the basis of the treatment of the control group.The treatment cycle was 2 months,and the follow-up was 4 months.The therapeutic effect,incidence of complications,best corrected visual acuity and macular center retinal thickness before and after treatment,and the number of retreatments due to DME within 4 months after treatment were compared between two groups.Results The therapeutic effect of the observation group was better than that of the control group(P<0.05).There was no statistically significant difference in the total incidence of complications between two groups(P>0.05).After treatment,the best corrected visual acuity of two groups was improved significantly,and the observation group was better than the control group(P<0.001);the macular center retinal thickness of two groups was significantly reduced,and the observation group was better than the control group(P<0.001).Within 4 months after treatment,compared with the control group,the observation group had fewer repeated treatments for DME(P<0.05).Conclusion The combination of self-made traditional Chinese compound and conbercept has the more significant therapeutic effect on DME,which can effectively improve the visual acuity of patients,reduce the degree of macular edema,decrease the number of intraocular injections,and reduce the recurrence rate.This therapy has clinical application value.
