摘 要:目的系统评价血脂水平与突发性聋(sudden hearing loss,SHL)的相关性。方法采用主题词、自由词和关键字组合,规范检索国内外各大数据库收集的有关血脂水平与SHL相关性的病例对照研究,检索期限为建库至2022年8月。采用RevMan 5.3软件进行Meta分析。结果共纳入31篇文献。SHL组血清总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)水平均高于对照组,差异均有统计学意义(P<0.05);SHL组血清高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)与对照组比较,差异无统计学意义(P>0.05)。纳入文献发表时间、地域、SHL组年龄、SHL组例数、心脑血管疾病可能是影响血清TC、TG、LDL-C与SHL相关性的重要因素,差异有统计学意义(P<0.05);亚组分析结果显示,不同地域血清TC、TG、LDL-C水平与SHL明显相关,差异有统计学意义(P<0.05)。结论血清TC、TG、LDL-C水平可能与SHL发病相关,且受地域因素影响。血脂异常的识别有利于SHL风险的早期评估和管理。Objective To systematically evaluate the correlation between blood lipid levels and sudden hearing loss(SHL).Methods A standardized search of case-control studies on the correlation between lipid levels and SHL collected in major domestic and international databases was performed using a combination of subject terms,free words and keywords,with a search period from database creation to August 2022.Meta-analysis was performed using the RevMan 5.3 software.Results A total of 31 studies were included.Analysis showed that serum total cholesterol(TC),triglyceride(TG)and low-density lipoprotein cholesterol(LDL-C)levels were higher in the SHL group than in the control group(P<0.05);whereas serum HDL-C levels were not significantly different(P>0.05).Meta-regression analysis suggested that all or more factors including the time of publication of the included literature,geographical region,age of the study group,sample size and cardiovascular disease were important factors affecting the correlation between serum TC,TG and LDL-C and SHL(P<0.05).Subgroup analysis showed that the correlation between serum TC,TG and LDL-C levels and SHL was significantly different in different geographical areas(P<0.05).Conclusion Serum TC,TG and LDL-C levels may be associated with the development of SHL and are influenced by geographical factors.Identification of dyslipidemia facilitates the early assessment and management of SHL risks.
