摘  要:目的:探究计算机体层成像(CT)联合磁共振成像(MRI)对糖尿病足骨质及血管改变早期的诊断价值。方法:选择2021年1月~2023年5月在本院治疗的102例糖尿病足患者,患者均做CT和MRI检查,数字减影血管造影(DSA)结果为“金标准”,比较CT和MRI单独和联合检测对糖尿病足骨质的检出情况及对血管改变的诊断结果。结果:CT和MRI联合检测对骨质破坏、骨质疏松、软组织病变、病理性骨折的检出率高于CT和MRI检测(P<0.05);1408段血管中,DSA检出560段狭窄,848段无狭窄;联合检测检出552段狭窄,856段无狭窄,与“金标准”一致性较好(Kappa=0.960),优于CT和MRI单独检测(Kappa值分别为0.810,0.860);联合检测对糖尿病足患者下肢血管狭窄病变诊断的准确度、灵敏度、特异度高于CT和MRI单独检测,漏诊率、误诊率低于单独检测(P<0.05);联合检测对糖尿病足患者下肢血管狭窄程度诊断的准确率高于CT和MRI单独检测(P<0.05)。结论:CT和MRI联合检测能够提高糖尿病足患者骨质病变的检出率,同时对下肢血管狭窄的诊断结果与金标准一致性更高,可以提高诊断效能,而且联合检测在下肢血管狭窄程度的诊断中具有更高的准确性。Objective:To explore the diagnostic value of computed tomography(CT)combined with magnetic resonance imaging(MRI)in the early stage of bone and vascular changes in diabetes foot.Methods:The 102 patients with diabetic foot treated in our hospital from January 2021 to May 2023 were examined by CT and MRI,and the results of digital subtraction angiography(DSA)were the“gold standard”to compare the detection of CT and MRI on the diagnosis of diabetic foot bone and vascular changes.Results:The detection rate of CT and MRI for bone destruction,osteoporosis,soft tissue lesions and pathological fractures was higher than CT and MRI(P<0.05).Among 1408 segments of blood vessels,DSA detected 560 segments of stenosis and 848 segments of no stenosis;The joint detection detected 552 segments of stenosis,while 856 segments showed no stenosis,which was consistent with the“gold standard”(Kappa=0.960)and better than CT and MRI detection alone(Kappa values of 0.810 and 0.860,respectively);The accuracy,sensitivity and specificity of combined detection for the diagnosis of lower limb vascular stenosis in patients with diabetes foot were higher than that of CT and MRI alone,and the rate of missed diagnosis and misdiagnosis was lower than that of single detection,with a statistically significant difference(P<0.05).The combined test was more accurate in detecting the degree of lower limb vascular stenosis in diabetic foot patients than CT and MRI alone(P<0.05).Conclusion:The combined detection of CT and MRI can improve the detection rate of osteopathy in patients with diabetes foot.At the same time,the diagnosis results of lower limb vascular stenosis are more consistent with the gold standard,which can improve the diagnostic efficiency.Moreover,the combined detection has higher accuracy in the diagnosis of the degree of lower limb vascular stenosis,which can improve the diagnostic efficiency,and the combined detection has higher accuracy in the diagnosis of lower limb vascular stenosis.
