摘 要:目的探究同指指动脉转位术在断指再植中对断指的神经功能以及关节功能恢复的作用效果。方法以2021年3月至2023年10月商丘仁和中医院收治的86例断指患者进行研究,患者按照手术方式分为两组,其中44例患者开展自体小静脉桥接移植手术(对照组),42例患者开展同指指动脉转位术(研究组)。对比两组患者的基本资料、住院时间、手术时间差异;在治疗后1个月及6个月时评估患指关节功能水平;并在治疗前及治疗后1个月及6个月时评估患指主观功能水平[使用密歇根手功能量表(MHQ)评估]以及患指感觉功能,同时检测患指的温度以及血流速度。结果两组基本资料以及手术时间比较差异无统计学意义(P>0.05),研究组住院时间短于对照组,差异具有统计学意义(P<0.05),治疗1个月以及治疗6个月时,研究组患者的患指关节功能治疗有效率均高于对照组,差异均有统计学意义(P<0.05)。治疗前,两组患者MHQ量表得分、患指温度、患者血流速度及患指感觉功能评估结果对比差异无统计意义(P>0.05)。治疗后,两组MHQ量表得分、患指温度、血流速度、患指感觉功能评估得分均高于治疗前,且随着治疗后时间越长,MHQ量表得分、患指温度、血流速度、患指感觉功能评估得分增加明显,而治疗1个月及治疗6个月时,研究组患者MHQ量表得分、患指温度、血流速度、患指感觉功能评估得分均高于对照组,差异均有统计学意义(P<0.05)。结论在断指再植过程中应用同指指动脉转位术,对于断指神经功能以及关节功能的恢复有明显作用,能够促进患者断指再植后患指康复。Objective To explore the effects of same-finger artery transposition on nerve function and joint function recovery in finger replantation.Methods This study included 86 patients with finger amputations treated at Shangqiu Renhe Hospital of Traditional Chinese Medicine from March 2021 to October 2023.Patients were divided into two groups based on surgical method 44 patients underwent autologous vein bridge transplantation(control group),and 42 patients underwent same-finger artery transposition(study group).Basic data,hospitalization time,and operation time differences between the two groups were compared.Joint function levels were evaluated at 1 month and 6 months after treatment.Subjective functional levels of the affected fingers were assessed using the Michigan Hand Outcomes Questionnaire(MHQ),and sensory function was evaluated.Temperature and blood flow velocity of the affected fingers were also measured before treatment and at 1 month and 6 months after treatment.Results There were no significant differences in basic data and operation time between the two groups(P>0.05).The study group had a shorter hospitalization time than the control group(P<0.05).At 1 month and 6 months after treatment,the effective rate of joint function treatment in the study group was higher than that in the control group(P<0.05).Before treatment,there were no significant differences in MHQ scores,finger temperature,blood flow velocity,and sensory function between the two groups(P>0.05).After treatment,MHQ scores,finger temperature,blood flow velocity,and sensory function scores were higher in both groups,with significant improvements over time,and the study group showed better results than the control group(P<0.05).Conclusion The application of same-finger artery transposition in finger replantation has a significant effect on the recovery of nerve function and joint function in amputated fingers,promoting the rehabilitation of the affected fingers after replantation.
