64排CT检查在胃癌患者胃部病变诊断及转移复发监测中的应用

摘　　要：目的:分析64排CT检查在胃癌患者胃部病变诊断及转移复发监测中的应用价值。方法:选取经术后病理学检查确诊的70例胃癌患者,均为大连市第四人民医院2021年1月~2022年12月所收治,入院后全部患者均接受64排CT检查,并将病理检查结果作为金标准,对非转移性淋巴结、转移性淋巴结的扫描参数、长短径及主要血流动力学参数进行比较。结果:病理检查结果显示,全部70例患者中,出现淋巴结转移的患者共52例,未出现淋巴结转移的患者共有18例;术中操作时取出的淋巴结共有490枚,转移性、非转移分别有348枚、142枚。和非转移性淋巴结相比较,转移性淋巴结的各项扫描参数、血流量更大(P<0.05),而表面通透性则更高(P<0.05)。CT检查的灵敏度为92.31%(48/52),特异度为88.89%(16/18),符合率为91.43%(64/70)。结论:在胃癌患者胃部病变诊断及转移复发监测中,应用64排CT检查具有非常重要的作用,其不仅具有较高的诊断灵敏度和特异度,而且具有较高的N分期诊断准确率。Objective:To analyze the application value of 64 slice CT examination in the diagnosis of gastric lesions and monitoring of metastasis and recurrence in gastric cancer patients.Methods:70 gastric cancer patients confirmed by postoperative pathological examination were selected,all of whom were admitted to the Fourth People’s Hospital of Dalian City from January 2021 to December 2022.After admission,all patients underwent 64 slice CT examination,and the pathological examination results were used as the gold standard.The postoperative pathological examination results were used as the gold standard to compare the scanning parameters,long and short diameters,and main hemodynamic parameters of non metastatic and metastatic lymph nodes.Results:The pathological examination results showed that among all 70 patients,a total of 52 had lymph node metastasis and 18 had no lymph node metastasis;During the operation,a total of 490 lymph nodes were removed,including 348 metastatic and 142 non metastatic lymph nodes.Compared with non metastatic lymph nodes,metastatic lymph nodes had higher scanning parameters and blood flow(P<0.05),while their surface permeability was higher(P<0.05).The sensitivity of CT examination was 92.31%(48/52),the specificity was 88.89%(16/18),and the coincidence rate was 91.43%(64/70).Conclusion:The application of 64 slice CT plays a very important role in the diagnosis of gastric lesions and monitoring of metastasis and recurrence in gastric cancer patients.It not only has high diagnostic sensitivity and specificity,but also has high accuracy in N-stage diagnosis.
