摘 要:目的探讨高比例与低比例新鲜冰冻血浆联合去白细胞悬浮红细胞用于严重内科失血患者的效果及对其纤溶功能的影响。方法回顾性分析。抽取2020年10月至2023年10月安康市中心医院收治的严重内科失血患者96例, 根据数字奇偶数分为A、B组, 每组48例。两组患者均给予新鲜冰冻血浆联合去白细胞悬浮红细胞治疗, 其中A组给予低比例新鲜冰冻血浆, B组给予高比例新鲜冰冻血浆。比较两组临床指标、纤溶功能[凝血酶原时间(PT)、D-二聚体(D-D)、活化部分凝血活酶原时间(APTT)]、免疫功能(T淋巴细胞亚群CD4^(+)、CD8^(+)占比及CD4^(+)/CD8^(+))以及不良反应。结果 B组休克纠正时间、血容量恢复时间及重症监护室收治时间均少于A组(P均<0.05)。治疗后, 两组PT高于治疗前, D-D、APTT低于治疗前, 且B组PT高于A组, D-D、APTT低于A组(P<0.05)。治疗后, 两组CD4^(+)、CD4^(+)/CD8^(+)均高于治疗前, CD8^(+)低于治疗前, 且B组CD4^(+)、CD4^(+)/CD8^(+)高于A组, CD8^(+)低于A组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论相较于低比例新鲜冰冻血浆联合去白细胞悬浮红细胞治疗, 高比例配比可有效改善严重内科失血患者的临床指标, 调节其纤溶功能。Objective To investigate the effects of high-ratio and low-ratio fresh frozen plasma combined with leukoreduced red blood cells on patients with severe internal medicine hemorrhage,and to analyze its influence on fibrinolysis function.Methods A retrospective analysis was conducted on 96 patients with severe internal medicine hemorrhage admitted to Ankang Central Hospital from October 2020 to October 2023.And they were divided into groups A and B according to odd and even numbers,with 48 cases in each group.Both groups of patients were treated with fresh frozen plasma combined with leukoreduced red blood cells.Group A received low-ratio fresh frozen plasma,while group B received high-ratio fresh frozen plasma.Clinical indicators,fibrinolysis function assessed by prothrombin time(PT),D-dimer(D-D)and activated partial thromboplastin time(APTT),immune function assessed by proportions of T lymphocyte subsets including cluster of differentiation 4^(+)(CD4^(+)),cluster of differentiation 8^(+)(CD8^(+))and CD4^(+)/CD8^(+),and adverse reactions of the two groups were compared.Results The shock correction time,blood volume recovery time,and intensive care unit admission time in the group B were all shorter than those in the group A(all P<0.05).After treatment,PT in both groups were higher than those before treatment,while D-D and APTT in both groups were lower than those before treatment(P<0.05);the group B had higher PT,lower D-D and lower APTT compared with the group A(P<0.05).After treatment,CD4^(+)and CD4^(+)/CD8^(+)in both groups were higher than those before treatment,and CD8^(+)was lower than that before treatment,in addition,CD4^(+)and CD4^(+)/CD8^(+)in the group B were higher than those in the group A,while CD8^(+)was lower than that in the group A(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Compared with low-ratio fresh frozen plasma combined with leukoreduced red blood cells therapy,high-ratio fresh frozen plasma proto
