卵巢子宫内膜异位囊肿不孕患者腹腔镜术后妊娠的影响因素及EFI评分的预测价值

摘　　要：目的分析卵巢子宫内膜异位囊肿不孕患者腹腔镜术后妊娠的影响因素以及生育指数(EFI)评分对其术后妊娠的预测价值。方法回顾性抽取2019年1月至2020年5月在河南省人民医院接受腹腔镜手术治疗的104例卵巢子宫内膜异位囊肿不孕患者作为研究对象,术后对其进行为期3年的随访,根据术后随访结果将其分为妊娠成功组(68例)和妊娠失败组(36例)。比较两组一般资料、疾病情况、手术情况以及术后EFI评分,分析影响术后妊娠的因素,并分析EFI评分的预测价值。结果年龄≥30岁、最大囊肿直径>5 cm、不孕年限>3年、不孕类型为继发性、EFI评分<7分属于卵巢子宫内膜异位囊肿不孕患者腹腔镜术后妊娠的危险因素(OR>1,P<0.05)。妊娠成功组EFI评分高于妊娠失败组(P<0.05),且经受试者工作特征曲线分析,EFI评分预测卵巢子宫内膜异位囊肿不孕患者腹腔镜术后妊娠成功的价值良好(P<0.05)。结论年龄和囊肿直径大、不孕年限长、继发性不孕及术后EFI评分低是卵巢子宫内膜异位囊肿不孕患者腹腔镜术后妊娠失败的高危因素,且术后通过评估EFI评分可预测其术后妊娠成功率。Objective To analyze the influencing factors of pregnancy after laparoscopic surgery in infertile patients with ovarian endometrial cysts,and to investigate the predictive value of endometriosis fertility index(EFI)score for postoperative pregnancy.Methods A total of 104 infertile patients with ovarian endometrial cysts who underwent laparoscopic surgery in Henan Provincial People’s Hospital from January 2019 to May 2020 were selected as research objects retrospectively,and they were followed up for 3 years after surgery.The selected patients were divided into the pregnancy success group and the pregnancy failure group according to the results of the postoperative follow-up.The general information,disease conditions,surgical conditions,and postoperative EFI score of the two groups were compared to analyze the factors affecting the pregnancy in the postoperative period;and the predictive value of EFI score was analyzed.Results Age≥30 years,cyst with major diameter>5 cm,infertility duration>3 years,secondary infertility,and EFI score<7 were risk factors for pregnancy after laparoscopic surgery in infertile patients with ovarian endometrial cysts(OR>1,P<0.05).The EFI score in the pregnancy success group was higher than that in the pregnancy failure group(P<0.05).The receiver operating characteristic curve revealed that EFI score had great significance in predicting pregnancy after laparoscopic surgery in infertile patients with ovarian endometrial cysts(P<0.05).Conclusions Higher age,large cyst diameter,longer years of infertility,secondary infertility,and lower postoperative EFI scores are high-risk factors for pregnancy failure after laparoscopic surgery in infertile patients with ovarian endometrial cysts.And the probability of postoperative pregnancy success can be predicted by evaluating EFI scores after surgery.
