加味玉屏风汤联合西药治疗对变应性鼻炎肺脾气虚证患者血清炎性因子及中医证候的影响

摘　　要：目的探讨加味玉屏风汤联合西药治疗变应性鼻炎(AR)肺脾气虚证的临床效果。方法队列研究。抽取2021年1月至2023年12月周口市第一人民医院收治的86例AR患者,按随机数字表法分为对照组和观察组,每组43例。对照组予以西药治疗,观察组加用加味玉屏风汤治疗,持续治疗4周。比较两组临床疗效、血清炎性因子[白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平、中医证候积分及不良反应发生情况。结果观察组总有效率(95.35%,41/43)高于对照组(81.40%,35/43),P<0.05。治疗后,观察组血清IL-4、IL-6、TNF-α、hs-CRP水平均低于对照组(P均<0.05)。治疗后,观察组中医证候积分均低于对照组(P均<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论加味玉屏风汤联合西药治疗可提高AR肺脾气虚证患者的治疗效果,加快疾病症状消失,降低炎性因子水平,安全可行。Objective To investigate the clinical efficacy of modified Yupingfeng decoction combined with western medicine in the treatment of patients with allergic rhinitis(AR)complicated by lung-spleen qi deficiency.Methods A total of 86 patients with AR complicated by lung-spleen qi deficiency in the Zhoukou First People’s Hospital from January 2021 to December 2023 were selected for the cohort study,and they were divided into a control group and an observation group by random number table method,with 43 cases in each group.The control group was treated with western medicine,while the observation group was treated with modified Yupingfeng decoction additionally.Both groups were treated for 4 weeks.The clinical efficacy,levels of serum inflammatory factors,including interleukin-4(IL-4),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and high-sensitivity C-reactive protein(hs-CRP),traditional Chinese medicine(TCM)syndrome scores and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.35%(41/43),higher than the 81.40%(35/43)of the control group(P<0.05).After treatment,the levels of IL-4,IL-6,TNF-α,hs-CRP in the observation group were lower than those in the control group(all P<0.05).After treatment,the TCM syndrome scores in the observation group were lower than those in the control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions The combination of modified Yupingfeng decoction and western medicine can improve the therapeutic effect of patients with AR complicated by lung-spleen qi deficiency,accelerate the disappearance of disease symptoms,and reduce levels of inflammatory factors,with feasible safety.
