肺泡灌洗联合纤维支气管镜吸痰治疗重症肺炎合并呼吸衰竭的效果

摘　　要：目的探讨肺泡灌洗联合纤维支气管镜吸痰治疗重症肺炎合并呼吸衰竭的效果。方法选择内蒙古科技大学包头医学院第二附属医院2019年6月—2023年12月收治的108例重症肺炎合并呼吸衰竭患者作为研究对象。所有患者根据治疗方案的不同分为研究组58例和对照组50例。研究组实施纤维支气管镜吸痰联合肺泡灌洗治疗,对照组实施无菌吸痰管吸痰治疗。比较两组治疗后的疗效、临床相关指标,以及治疗前后肺部感染评分及炎性因子水平。结果研究组临床总有效率96.55%,高于对照组的82.00%,差异有统计学意义(P<0.05)。治疗后研究组临床相关指标中咳嗽缓解时间、肺部啰音缓解时间、发热缓解时间、机械通气时间、抗菌药物使用时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,研究组肺部感染评分及炎性因子水平均低于治疗前,且治疗后研究组上述指标均优于对照组,差异有统计学意义(P<0.05)。结论肺泡灌洗联合纤维支气管镜吸痰治疗重症肺炎合并呼吸衰竭,可有效改善机体炎症水平和肺部感染情况,临床疗效更为理想,值得推广使用。Objective To explore the effect of alveolar lavage combined with fiberoptic bronchoscopy for sputum aspiration in the treatment of severe pneumonia with respiratory failure.Methods A total of 108 patients of severe pneumonia with respiratory failure admitted to the Second Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science and Technology from June 2019 to December 2023 were selected as the research subjects.All patients were divided into a research group of 58 cases and a control group of 50 cases based on different treatment plans.The research group received fiberoptic bronchoscopy combined with bronchoalveolar lavage treatment,while the control group received sterile suctioning treatment.Compare the efficacy,clinical related indicators,as well as pulmonary infection scores and inflammatory factor levels before and after treatment between two groups.Results The total clinical effective rate of the research group was 96.55%,which was significantly higher than the control group’s 82.00%(P<0.05).After treatment,the clinical indicators related to cough relief time,lung rale relief time,fever relief time,mechanical ventilation time,and antibiotic use time in the research group were all significantly shorter than those in the control group(P<0.05).After treatment,the lung infection score and inflammatory factor levels in the research group were significantly lower than before treatment,and the above indicators in the research group were significantly better than those in the control group(P<0.05).Conclusion The combination of alveolar lavage and fiberoptic bronchoscopy for sputum aspiration in the treatment of severe pneumonia complicated with respiratory failure can effectively improve the level of inflammation and pulmonary infection,and has a more ideal clinical effect,which is worth promoting and using.
