当归饮子加味联合糠酸莫米松软膏治疗早期非小细胞肺癌术后奥希替尼相关性皮肤干燥瘙痒的临床研究

摘　　要：目的:观察当归饮子加味联合糠酸莫米松软膏治疗早期非小细胞肺癌(NSCLC)术后奥希替尼相关性皮肤干燥瘙痒的临床疗效。方法:将肿瘤分期系统(TNM)IB~IIIA期的NSCLC术后接受奥希替尼治疗后出现符合血虚风燥型皮肤干燥瘙痒的60例患者随机分为观察组和对照组:观察组口服当归饮子加味联合糠酸莫米松软膏外用,对照组单用糠酸莫米松软膏治疗。两组治疗4个疗程(7d为1个疗程),比较两组治疗前后患者皮肤干燥、瘙痒症状积分及疗效、皮肤干燥瘙痒等级及皮肤病生活质量指数量表(DLQI)评分变化情况。结果:治疗后两组皮肤瘙痒症状积分较治疗前均下降,其中观察组除皮损面积外,其余积分均低于对照组(P<0.05,P<0.01);治疗后两组皮肤干燥症状积分较治疗前均有下降且观察组评分均优于对照组(P<0.01);治疗后两组皮肤干燥瘙痒等级均下降,且观察组低于对照组(P<0.05);治疗后两组DLQI评分均下降,且观察组低于对照组(P<0.05,P<0.01)。结论:当归饮子加味联合糠酸米松软膏治疗早期NSCLC术后奥希替尼相关性血虚风燥型皮肤干燥瘙痒疗效确切,可显著改善患者皮肤干燥及瘙痒症状,提高患者生活质量,提升患者依从性。Objective:To observe the clinical effect of modified Danggui Yin combined with mometasone furoate ointment in the treatment of skin dryness and pruritus associated with osimertinib after operation for early non-small cell lung carcinoma(NSCLC).Methods:A total of 60 TNM stage IB~IIIA patients after NSCLC who developed dry and pruritus skin caused by blood deficiency and wind-dryness after osimertinib treatment were randomly divided into observation group and control group.Patients in the observation group were treated with modified Danggui external use of mometasone furoate ointment,while patients in the control group were treated with mometasone furoate ointment only.The two groups were treated for 4 courses(7 days for 1 course).Then,the score and efficacy of skin dryness and pruritus symptom,the grade of skin dryness and pruritus and the score of Dermatology Life Quality Index(DLQI)were compared between the two groups before and after treatment.Results:After treatment,the scores of pruritus symptoms in both groups were lower than those before treatment,and the scores in the observation group were lower than those in the control group except for the skin lesion area(P<0.05,P<0.01).After treatment,the scores of skin dryness symptoms in the two groups were lower than those before treatment,and the scores in the observation group were better than those in the control group(P<0.01).After treatment,the degree of skin dryness and pruritus in the two groups decreased,and the degree in the observation group was lower than that in the control group(P<0.05).After treatment,DLQI scores of both groups decreased,and the score in the observation group was lower than that in the control group(P<0.05,P<0.01).Conclusion:In the treatment of skin dryness and pruritus caused by blood deficiency and wind-dryness associated with osimertinib after operation of early non-small cell lung carcinoma,the combined effect of modified Danggui Yin and mometasone furoate ointment is accurate,which can significantly alleviate the symptoms of skin
