黄连温胆汤加减联合丹红注射液用于痰热扰心型房颤患者射频消融术后的疗效

摘　　要：目的:分析黄连温胆汤加减联合丹红注射液用于痰热扰心型房颤患者射频消融术后的疗效。方法:回顾性分析103例2021年1月—2023年12月在天津中医药大学第一附属医院行射频消融术的痰热扰心型房颤患者的临床资料,按治疗方法分成常规组与联合组,常规组51例,联合组52例。两组均给予常规西医治疗,常规组使用丹红注射液静脉注射,联合组在此基础上加用黄连温胆汤加减口服治疗。对两组患者治疗前后的中医证候积分、心功能与血清学指标水平、临床疗效与不良反应进行观察。结果:治疗前,两组主症与次症的中医证候积分、左心室射血分数(LVEF)、左心房内径(LAD)、舒张末期室间隔厚度(IVST)、心肌肌钙蛋白I(cTnI)、N-端脑钠肽前体(NT-proBNP)、血管紧张素Ⅱ(AngⅡ)及超敏C反应蛋白(hsCRP)水平比较,差异均无统计学意义(P>0.05)。治疗后,联合组中医证候积分、cTnI、NT-proBNP、AngⅡ、hsCRP水平、LAD和IVST均低于常规组(P<0.05);联合组总治疗有效率与LVEF均高于常规组(P<0.05);不良反应总发生率方面,两组差异不明显(P>0.05)。结论:黄连温胆汤加减联合丹红注射液用于痰热扰心型房颤患者射频消融术后可有效缓解临床症状,改善心功能,抑制炎症反应,提高临床疗效。Objective:To analyze the efficacy of modified Huanglian Wendan Decoction combined with Danhong Injection for patients with phlegm-heat disturbing the heart type atrial fibrillation(AF)after radiofrequency ablation.Methods:A retrospective analysis was conducted on the clinical data of 103 patients with AF of the phlegm-heat disturbing the heart type who underwent radiofrequency ablation at The First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from January 2021 to December 2023.The patients were divided into a conventional group(51 cases)and a combined group(52 cases)according to the treatment method.Both groups received conventional Western medicine treatment;the conventional group was treated with Danhong Injection intravenously,while the combined group received oral modified Huanglian Wendan Decoction in addition to Danhong Injection.TCM syndrome scores,cardiac function,serum index levels,clinical efficacy,and adverse reactions before and after treatment were observed in both groups.Results:Before treatment,there were no significant differences between the two groups in terms of TCM syndrome scores for primary and secondary symptoms,left ventricular ejection fraction(LVEF),left atrial diameter(LAD),end-diastolic interventricular septal thickness(IVST),cardiac troponin I(cTnI),N-terminal pro-brain natriuretic peptide(NT-proBNP),angiotensin II(Ang II),and high-sensitivity C-reactive protein(hsCRP)(P>0.05).After treatment,the combined group showed lower TCM syndrome scores,cTnI,NT-proBNP,Ang II,hsCRP levels,LAD,and IVST than the conventional group(P<0.05).The combined group also had higher overall treatment efficacy and LVEF(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Modified Huanglian Wendan Decoction combined with Danhong Injection effectively alleviates clinical symptoms,improves cardiac function,suppresses inflammatory responses,and enhances clinical efficacy in patients with phlegm-heat disturbi
