手法复位联合外固定夹板治疗尺桡骨骨折的疗效及关节功能分析

摘　　要：目的探讨手法复位联合外固定夹板治疗尺桡骨骨折患者的疗效及对其关节功能的影响。方法简单随机选取2020年5月—2022年9月德化县中医院确诊为尺桡骨骨折的患者70例为研究对象,根据不同治疗方法分为两组,每组35例。对照组行切开复位内固定处理,观察组行手法复位联合外固定夹板处理,比较两组临床疗效、影像学参数、腕关节功能和并发症。结果观察组治疗总有效率为80.00%(28/35),高于对照组的57.14%(20/35),差异有统计学意义(χ^(2)=4.242,P=0.039)。治疗后,观察组3项影像学参数(掌倾角、尺偏角以及桡骨相对高度)均优于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组Cooney腕关节功能评估表4个维度(功能状况、握力、疼痛及活动度)得分均高于对照组,差异有统计学意义(P均<0.05)。两组并发症总发生率对比,差异无统计学意义(P>0.05)。结论手法复位联合外固定夹板治疗能够提升尺桡骨骨折患者的治疗效果,能够更有效改善腕关节功能及影像学参数,且并发症较少,可保障治疗安全。Objective To investigate the effect of manual reduction combined with external fixation splint in the treat⁃ment of patients with ulnar and radial fractures and its influence on joint function.Methods From May 2020 to Sep⁃tember 2022,70 patients with ulnar and radial fractures diagnosed by Dehua County Hospital of Traditional Chinese Medicine were simple randomly selected as the research objects.According to different treatment methods,they were divided into two groups,with 35 cases in each group.The control group was treated with open reduction and internal fixation,and the observation group was treated with manual reduction combined with external fixation splint.The clini⁃cal efficacy,imaging parameters,wrist function and complications of the two groups were compared.Results The total effective rate of the observation group was 80.00%(28/35),which was higher than 57.14%(20/35)of the control group,and the difference was statistically significant(χ^(2)=4.242,P=0.039).After treatment,the three imaging param⁃eters(palmar tilt angle,ulnar deviation angle and relative height of radius)in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the scores of four dimensions(functional status,grip strength,pain and activity)of Cooney Wrist Function Assessment Scale in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion The treatment of manual reduction combined with external fixation splint can improve the therapeutic ef⁃fect of patients with ulnar and radial fractures,can more effectively improve the wrist joint function and imaging parameters,and has fewer complications,which can ensure the safety of treatment.
