摘 要:目的 分析膝关节软骨退变关节镜微骨折术与自体富血小板血浆的治疗效果。方法 方便选取2022年7月—2023年11月扬州洪泉医院收治的207例膝关节软骨退变患者为研究对象,根据不同治疗方法分为手术组(n=104)、研究组(n=103)。手术组行关节镜微骨折术治疗,研究组另行自体富血小板血浆疗法,对比两组患者的膝关节活动度、疼痛介质水平、膝关节功能以及炎性因子水平。结果 治疗后,研究组膝关节屈曲度(118.46±11.26)°、伸直度(-3.20±1.18)°、屈伸弧度(123.31±13.12)°均优于手术组的(102.30±9.41)°、(-6.29±1.37)°、(111.92±11.36)°,差异有统计学意义(t=11.208、17.380、6.679,P均<0.05);前列腺素E2、应激蛋白、5-羟色胺、多巴胺水平均低于手术组,差异有统计学意义(P均<0.05);国际膝关节文献委员会评分、膝关节评分均高于手术组,差异有统计学意义(P均<0.05);白三烯B4、白三烯C4、白三烯D4、白细胞介素-1β、肿瘤坏死因子-α水平均低于手术组,差异有统计学意义(P均<0.05)。结论 膝关节软骨退变采用关节镜微骨折术与自体富血小板血浆治疗对炎性因子释放量有较好的抑制功效,可以改善膝关节功能与活动度。Objective To investigate the effect of arthroscopic microfracture and autologous platelet-rich plasma in the treatment of knee cartilage degeneration.Methods A total of 207 patients with knee cartilage degeneration admit⁃ted to Yangzhou Hongquan Hospital from July 2022 to November 2023 were conveniently selected as the research ob⁃jects.According to different treatment methods,they were divided into operation group(n=104)and study group(n=103).The operation group was treated with arthroscopic microfracture,and the study group was treated with autologous platelet-rich plasma therapy.The knee joint mobility,pain mediator level,knee joint function and inflammatory factor level were compared between the two groups.Results After treatment,the knee flexion(118.46±11.26)°,extension(-3.20±1.18)°,flexion and extension arc(123.31±13.12)°in the study group were better than those in the operation group(102.30±9.41)°,(-6.29±1.37)°,(111.92±11.36)°,the differences were statistically significant(t=11.208,17.380,6.679,all P<0.05);the levels of prostaglandin E2,stress protein,5-hydroxytryptamine and dopamine in the operation group were lower than those in the operation group,and the differences were statistically significant(all P<0.05);the International Knee Documentation Committee score and knee joint score were higher than those in the operation group,and the differences were statistically significant(both P<0.05).The levels of leukotriene B4,leukotriene C4,leukotriene D4,interleukin-1βand tumor necrosis factor-αwere lower than those in the operation group,and the dif⁃ferences were statistically significant(all P<0.05).Conclusion Arthroscopic microfracture and autologous platelet-rich plasma have a good inhibitory effect on the release of inflammatory factors in knee joint cartilage degeneration,and can improve knee joint function and activity.
