摘 要:目的:观察电针预处理对急性肺损伤(ALI)大鼠炎症反应及核因子E2相关因子2(Nrf2)/血红素氧合酶-1(HO-1)通路的影响,探讨电针预处理在ALI中发挥的效应及可能机制。方法:将18只清洁级雄性Wistar大鼠随机分为对照组、脂多糖(LPS)组、电针预处理+LPS组,每组6只。电针预处理组予以电针预处理,疏密波,频率3 Hz/15 Hz,电流强度1~2 mA,30 min/次,1次/天,连续治疗5天。第5天,对照组麻醉后眼眶取血,再取肺组织;LPS组与电针预处理组尾静脉1/3处注射LPS溶液(注射剂量5 mg/kg,浓度5 mg/mL)6 h后取样。采用苏木素伊红染色法观察肺组织病理变化;光学显微镜观察并进行肺损伤病理评分;酶联免疫分析法检测IL-1β、IL-10水平;荧光定量PCR及Western blot分别检测Nrf-2、HO-1mRNA和蛋白表达情况。结果:与对照组比较,LPS组肺泡壁增厚,肺间质可见大量炎性细胞浸润,肺泡可见红细胞渗出;血清IL-1β水平升高,IL-10水平降低(P<0.01);Nrf-2和HO-1 mRNA及蛋白表达明显降低(P<0.01);与LPS组相比,电针预处理组肺组织损伤程度明显减轻;IL-1β水平明显降低(P<0.01),IL-10水平升高(P<0.05);Nrf2、HO-1 mRNA及蛋白表达明显增高(P<0.01,P<0.05),蛋白表达水平变化趋势与mRNA表达一致。结论:电针预处理对急性肺损伤大鼠炎症反应具有抑制作用,其对肺保护效应可能与促进Nrf2、HO-1分泌,降低肺内促炎因子水平、升高抑炎因子水平有关。Objective:To observe the effect of electroacupuncture(EA)pretreatment at"Zusanli"(ST 36)and"Chize"(LU 5)points on the inflammatory and erythroid 2-related factor 2/Heme oxygenase 1(Nrf-2/HO-1)pathway in rats with sepsis-induced acute lung injury(ALI),and to explore the protective effect and possible mechanism of EA pretreatment in ALI.Methods:According to the principle of randomization,18 Wistar male rats were equally divided into 3 groups,namely the control group,LPS model group and EA preconditioning+LPS model group.The ALI model was established by intraperitoneal injection of lipopolysaccharide(LPS).EA(3 Hz/15 Hz,1~2 mA)was applied to bilateral ST 36 and LU 5 for 30 min,once a day for 5 consecutive days before modeling.HE staining was used to observe the pathological changes of the lung,and the light microscopy was observed and the pathological score of lung injury was performed;the secretions of inflammatory factors IL-1βand IL-10 in the serum and lung tissue were measured by ELISA kits;and lung tissue Nrf-2/HO-1 pathway-related mRNA and protein expression were detected by PCR and Western Blot.Results:Compared with the control group,the alveolar wall of the model group was thickened,a large number of inflammatory cells were infiltrated in the lung interstitium,and red blood cell exudation was observed in the alveolar;serum IL-1βlevels were increased,and IL-10 levels were decreased(P<0.01).The mRNA expressions of Nrf-2 and HO-1 were decreased(P<0.01).Compared with the model group,the degree of lung injury in the electroacupuncture pretreatment+model group was significantly reduced.The levels of IL-1βwere decreased(P<0.01),while the levels of IL-10 were increased(P<0.05).The mRNA and protein expression levels of Nrf2 and HO-1 were significantly increased(P<0.01,P<0.05).The trend of protein expression was consistent with mRNA expression.Conclusions:EA pretreatment can reduce pulmonary inflammatory response in ALI rats,and its protective effect on the lung may be related to inhibiting the expression of Nrf2,pr
