摘 要:目的围绕慢性阻塞性肺病急性加重(AECOPD)并呼吸衰竭(RF)患者,分析无创机械通气(NIV)联合高流量氧疗(HFNC)效果及安全性。方法我院收治的AECOPD并RF患者若干例,2021年1月至2023年1月抽取80例,按照随机数字表法分为对照组(40例,NIV)与观察组(各42例,联合HFNC),比较两组临床疗效、肺功能、氧化应激、血气分析及炎症指标、不良反应。结果观察组临床疗效总有效率高于对照组(P<0.05),治疗后,观察组FEV1、FVC、PEF、PaO_(2)、PaCO_(2)、SaO_(2)、p H、PCT、CRP、IL-6改善优于对照组(P<0.05);与对照组不良事件发生率低于观察组(P<0.05)。结论在AECOPD并RF患者中实施NIV联合HFNC,可明显减轻患者气促、呼吸困难症状,改善肺功能,减轻炎症反应,降低不良反应的发生。Objective To analyze the efficacy and safety of non-invasive mechanical ventilation(NIV)combined with high-flow oxygen therapy(HFNC)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and respiratory failure(RF).Methods Several patients with AECOPD and RF admitted to our hospital were randomly divided into a control group(40 cases,NIV)and an observation group(42 cases each,combined with HFNC)based on a specific time period From January 2021 to January 2023.The clinical efficacy,lung function,oxidative stress,blood gas analysis,inflammatory indicators,and adverse reactions were compared between the two groups using a random number table method.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,FEV1,FVC,PEF,PaO_(2),PaCO_(2),SaO_(2),pH,PCT,CRP and IL-6 in the observation group were better than those in the control group(P<0.05).The incidence of adverse events in control group was lower than that in observation group(P<0.05).Conclusions Implementing NIV combined with HFNC in AECOPD and RF patients can significantly alleviate symptoms of dyspnea and difficulty breathing,improve lung function,alleviate inflammatory reactions,and reduce the occurrence of adverse reactions.
