摘 要:目的:探讨血管内皮生长因子(VEGF)、表皮生长因子受体(EGFR)、蛋白CD34和CD105表达水平对不同分期内痔病理变化的影响。方法:选取2021年6月至2023年12月,本院治疗的120例内痔患者,根据内痔分期分为Ⅱ期组35例,Ⅲ期组45例和Ⅳ期组40例。分别行免疫组化检测,观察VEGF、EGFR、CD34和CD105表达与内痔分期及微血管密度(MVD)的关系。结果:VEGF、EGFR、CD34和CD105在3组中均有表达,且随着内痔分期增加表达逐渐增强。其中Ⅲ期组和Ⅳ期组中VEGF、EGFR、CD34和CD105水平均明显高于Ⅱ期组(P<0.05),Ⅲ期组和Ⅳ期组比较,差异均无统计学意义(P>0.05)。通过对内痔MVD计数发现,Ⅲ期内痔最多,Ⅳ期次之,Ⅱ期最少。结论:随着内痔的发展,Ⅱ期至Ⅲ期,VEGF、EGFR、CD34和CD105蛋白表达及MVD逐渐增强,Ⅲ期至Ⅳ期其表达相对减弱。内痔的发病机制与血管内皮细胞增殖和新血管增生密切相关。Objective To investigate the effects of the expression levels of vascular endothelial growth factor(VEGF),epidermal growth factor receptor(EGFR),proteins CD34 and CD105 on the pathological changes of internal hemorrhoids in different stages.Methods Total 120 patients with internal hemorrhoids treated in our hospital from June 2021 to December 2023 were divided into stageⅡgroup(35 cases),stageⅢgroup(45 cases)and stageⅣgroup(40 cases)according to the staging of internal hemorrhoids.Immunohistochemical tests were performed to observe the relationship between the expression of VEGF,EGFR,CD34 and CD105,the staging of internal hemorrhoids and microvascular density(MVD).Results VEGF,EGFR,CD34 and CD105 were expressed in all three groups,and the expressions enhanced gradually with the increase of internal hemorrhoid staging.The levels of VEGF,EGFR,CD34 and CD105 in the stageⅢand stageⅣgroups were significantly higher than those in the stageⅡgroup(P<0.05),and there was no statistical significance between the stageⅢand stageⅣgroups(P>0.05).By counting the MVD of internal hemorrhoids,it was found that the most hemorrhoids were in stageⅢ,followed by stageⅣ,and stageⅡwas the least.Conclusion With the progression of internal hemorrhoids,the expression of VEGF,EGFR,CD34 and CD105 and MVD enhanced gradually from stageⅡto stageⅢ,while the expression reduced relatively from stageⅢto stageⅣ.The pathogenesis of internal hemorrhoids is closely related to the proliferation of vascular endothelial cells and the proliferation of new vessels.
