加味四君子汤联合卡托普利片对慢性肾小球肾炎脾肾气虚型患者的临床疗效

摘　　要：目的评价加味四君子汤联合卡托普利片对慢性肾小球肾炎(CGN)脾肾气虚型患者的疗效。方法选取2019年3月至2021年2月在浦江县中医院肾病科收治的80例CGN脾肾气虚型患者。随机分为对照组(卡托普利片)和加味四君子汤组(加味四君子汤联合卡托普利片),每组40例。比较两组患者治疗前后中医症候积分和实验室指标差异。结果两组患者治疗前的中医症候积分和各项积分、血压和各项实验室指标差异无统计学意义(P>0.05)。治疗后,加味四君子汤组中医症候积分、腰膝酸痛、疲倦乏力、面浮肢肿和纳少腹胀积分、收缩压和24小时尿蛋白显著低于对照组(P<0.05),血清白蛋白显著高于对照组(P<0.05)。两组患者治疗后的各项积分显著低于治疗前(P<0.05)。结论加味四君子汤联合卡托普利片能够显著改善CGN脾肾气虚型患者的中医症候积分、24 h尿蛋白定量和血清白蛋白水平,治疗脾肾气虚型CGN有独特优势。Objective Evaluate the effectiveness of combining modified Sijunzi decoction with captopril in the treatment of chronic glomerular nephritis(CGN)with syndrome of deficiency of spleen and kidney qi.Methods A total of 80 cases of CGN with syndrome of deficiency of spleen and kidney qi treated in the Nephrology Department of Pujiang Hospital of Traditional Chinese Medi‐cine from March 2019 to February 2021 were selected.All patients were randomly divided into two groups,control group(captopril tab‐let)and modified Sijunzi decoction group(combining modified Sijunzi decoction with captopril tablet),with 40 cases in each group.The differences of traditional Chinese medicine(TCM)syndrome scores and laboratory indexes before and after treatment were com‐pared between the two groups.Results There were no significant differences in TCM syndrome scores,other scores,blood pressure and laboratory indexes between the two groups before treatment(P>0.05).After treatment,the scores of TCM syndromes,waist and knee soreness,fatigue,swelling of face and limbs,as well as the scores of poor appetite and abdominal distension,systolic blood pres‐sure and 24-hour urinary protein in the modified Sijunzi decoction group were significantly lower than those in the control group(P<0.05).The serum albumin levelin the modified Sijunzi decoction groupwas significantly higher than those in the control group(P<0.05).The scores of the two groups were significantly lower after treatment than before treatment(P<0.05).Conclusion Modified Sijunzi decoction combined with captopril significantly improves TCM syn‐drome score,24-hour urinary protein quantity and serum albumin in CGN patients,and has unique advantages in treating CGN with syndrome of deficiency of spleen and kidney qi.
