摘 要:目的应用静息态功能磁共振成像技术对高海拔居民进行脑部局部一致性(ReHo)的研究,探讨慢性环境缺氧人群脑皮层活动的神经机制。方法纳入高海拔世居人群(研究组)及低海拔人群(对照组)各30例,所有被试者均进行简易精神量化(MMSE)、头颅常规平扫及高分辨3D-T1、血氧饱和依赖扫描。基于Matlab数学平台,利用DPABI软件进行图像处理,通过双样本t检验以对比两组全脑局部一致性差异脑区。提取ReHo值与血液血红蛋白、血氧饱和度、二氧化碳分压、氧分压、血小板进行相关性分析。结果两组人员MMSE值均大于29。t检验结果经过Alpahshim校正,结果显示高海拔组的双侧辅助运动皮层SMA_R(Brodmann 6)、SMA_L(Brodmann 8)脑区的ReHo值高于对照组(P<0.001)。另外,右侧楔前叶Precuneus_R(Brodmann 29)的ReHo值低于正常对照组(P<0.05)。SMA_L(Brodmann 8)功能的改变与氧分压有较低的正相关(r=0.373,P=0.043)。结论双侧辅助运动皮层的ReHo增高,右侧楔前叶神经元自发活动一致性降低,可能是高海拔人群缺氧后运动能力增加及视觉器官功能受损的神经机制之一。Objective Resting-state functional MRI(rs-fMRI)was used to study the cerebral regional homogeneity(ReHo)in high-altitude population to explore the neural mechanism of brain activity in people with chronic environmental hypoxia.Methods Thirty subjects from the high-altitude and low-altitude(control group)populations underwent Mini-Mental State Examination(MMSE),conventional non-contrast,high-resolution 3-D T1-weighted and blood oxygen level dependent MRI.Based on the Matlab mathematical platform,the DPABI software was used for image processing,and the two-sample t-test was used to compare the two groups of brain regions with local consistency differences.The correlations between ReHo value and blood hemoglobin,oxygen saturation,partial pressure of carbon dioxide,partial pressure of oxygen(PaO2)and platelets were analyzed.Results The MMSE values of the two groups were>29.The results of t-test were corrected by Alphasim and showed that the ReHo values of the Supp_Motor_Area_R(SMA_R,Brodmann area 6)and Supp_Motor_Area_L(SMA_L,Brodmann area 8)brain regions in the high-altitude group were significantly higher than those in the control group(P<0.001).In addition,the ReHo values of Precuneus_R(Brodmann area 29)were significantly lower than that of the control group(P<0.05).The change of SMA_L function was positively correlated with PaO2(r=0.373,P=0.043).Conclusion The increase of ReHo in bilateral supplementary motor areas and the decrease in the spontaneous activity consistency of the right precuneus neurons may be one of the neural mechanisms for the increase of exercise capacity and the impairment of visual organs and functions after hypoxia in high-altitude populations.
