摘　　要：目的:联通各分散区(县)医疗机构,形成区域护理服务网络,提升区(县)级医疗机构居家养老护理服务能力。方法:组建多部门、多学科工作团队,以县域医共体为载体构建具有区域联动、资源共享的“互联网+护理”交互平台,联通不同级别、不同区域的县域医共体成员单位,通过平台精准对接出院患者居家护理服务需求,实现居家护理患者全病程管理。结果:2021年5月至2023年12月,6家县域医共体单位入驻“互联网+护理”交互平台并顺利运行;服务半径扩大至180千米外,覆盖5个区县;牵头单位培训市内外医疗机构20余家,累计服务患者900余人次。结论:“互联网+护理”交互平台实现了县域医共体资源整合、服务同质,可促进基层医疗机构“互联网+护理”建设发展及服务水平提升。Objective To connect medical institutions in scattered districts(counties),form a regional nursing service network,and enhance the home-based elderly nursing service capabilities of medical institutions at district(county)level.Methods A multi-department and multidisciplinary team was set up to build an"Internet+nursing"interactive platform with regional linkage and resource sharing by using the county medical consortium as the carrier,connecting member units of the consortium at different levels and in different regions,accurately satisfying the needs for home care services for patients discharged from hospitals through the platform,which can realize the whole-process management of patients in home care.Results From May 2021 to December 2023,6 county medical consortium institutions were recruited into the"Internet+Nursing"interactive platform and were operating smoothly.The service radius has been expanded to 180 kilometers,covering 5 districts and counties.The leading hospital trained more than 20 medical institutions in and out of the city,serving more than 900 patients in total.Conclusion The"Internet+nursing"interactive platform has realised the integration of resource and homogeneity of service in county medical consortium,and promote the development of"Internet+Nursing"development and enhancement of service level in primary-level medical institutions.
