协同家属护理管理对老年髋关节置换术后深静脉血栓的影响

摘　　要：目的:探讨老年髋关节置换术术后采取协同家属护理管理模式对常见并发症深静脉血栓(DVT)的预防作用。方法:选择医院骨科收治的68例老年髋关节置换术手术患者,采用随机数表法分成对照组(34例)和协同组(34例)。对照组患者行一般护理干预与常规血栓防控模式,协同组患者在常规血栓防控基础上实施协同家属护理管理模式,护理中与其家属共同制定护理管理计划,观察并评估DVT风险,实施分级预防与功能锻炼等,比较围手术期两组患者DVT预防知识掌握与DVT发生情况、平均住院天数、疗效与护理满意度评分。结果:协同组患者DVT发生率明显低于对照组(P<0.05);与对照组相比较,协同组患者的DVT预防知识掌握程度明显增加,平均住院天数明显缩短,术后髋关节功能优良率明显升高,且协同组患者护理满意度评分也明显优于对照组(P<0.05)。结论:对于老年髋关节置换术患者采取协同家属护理管理模式不仅可显著降低DVT发生率,而且可以缩短平均住院日,提高疾病疗效和护理满意度。Objective:To investigate the preventive effect of collaborative family nursing management on common complications of deep vein thrombosis(DVT)in elderly patients after hip replacement.Methods:Sixty-eight cases of elderly hip arthroplasty patients admitted to the orthopedic department of the hospital were divided into the control group(34 cases)and the cooperative group(34 cases)by the randomized numerical table method.The control group received general nursing intervention and routine thrombosis prevention and control mode,while the cooperative group implemented coordinated family nursing management mode based on routine thrombosis prevention and control.During nursing,nursing management plan was jointly formulated with family members,DVT risk was observed,assessed,and graded prevention and functional exercise were implemented.The knowledge of DVT prevention and the occurrence of DVT,the average length of hospital stay,curative effect and nursing satisfaction were compared between the two groups in perioperative period.Results:The incidence of DVT in the cooperative group was significantly lower than that in the control group(P<0.05).Compared with the control group,the knowledge of DVT prevention in the cooperative group was significantly increased,the average length of hospitalization was significantly shortened,the rate of hip joint function was significantly increased,and the nursing satisfaction score was also significantly better than the control group(P<0.05).Conclusion:For elderly patients with hip replacement,coordinated family nursing management can not only significantly reduce the incidence of DVT,but also shorten the average length of stay,improve the curative effect and improve the satisfaction of disease nursing.
