缩宫素联合宫颈环扎术与乳胶导尿管球囊放置术治疗产后出血的效果分析

摘　　要：目的:分析缩宫素联合宫颈环扎术与乳胶导尿管球囊放置术治疗产后出血的效果。方法:选取2022年12月—2024年1月重庆市涪陵区妇幼保健院产科进行阴道分娩并发生产后出血的患者100例进行研究,按照随机数字表法分为对照组(n=50)与观察组(n=50)。对照组应用缩宫素联合麦角新碱治疗,观察组应用缩宫素联合宫颈环扎术与乳胶导尿管球囊放置术治疗。比较两组治疗指标、止血有效率、凝血功能指标、血常规指标。结果:观察组患者止血时间、住院时间、下床活动时间均短于对照组(P<0.001);观察组止血有效率高于对照组(P=0.022);治疗后,两组活化部分凝血活酶时间、凝血酶原时间、凝血酶时间延长,纤维蛋白原水平降低,观察组优于对照组(P<0.05);治疗后24 h,两组血红蛋白水平高于治疗前,白细胞计数低于治疗前,观察组优于对照组(P<0.05);治疗后48 h,两组血红蛋白高于治疗后24 h,白细胞计数低于治疗后24 h,观察组优于对照组(P<0.05)。结论:缩宫素联合宫颈环扎术与乳胶导尿管球囊放置术治疗产后出血的应用效果良好,可提高止血效率,改善凝血功能指标及血常规指标,促进产后恢复。Objective:To analyze the effect of uterine hormone combined with cervix cercligation and latex catheter balloon placement in the treatment of postpartum hemorrhage.Methods:A total of one hundred patients with vaginal delivery and postpartum hemorrhage in the obstetrics department of Chongqing Fuling District Maternal and Child Health Hospital from December 2022 to January 2024 were selected as the study objects,and were randomly divided into control group(n=50)and observation group(n=50).The control group was treated with oxytocin combined with ergoclavine,and the observation group was treated with oxytocin combined with cervicalysis and latex catheter balloon placement.The therapeutic indexes,hemostatic efficiency,coagulation function indexes and blood routine indexes were compared between the two groups.Results:Hemostasis time,hospital stay time and getting out of bed time in observation group were shorter than those in control group(P<0.001);the hemostatic effective rate of observation group was higher than that of control group(P=0.022);after treatment,the activated partial thromboplastin time,prothrombin time and thrombin time of the two groups were prolonged,and the fibrinogen level was decreased,and the observation group was better than the control group(P<0.05);24 h after treatment,hemoglobin level was higher than before treatment,white blood cell count was lower than before treatment,observation group was better than control group(P<0.05);48 h after treatment,the hemoglobin levels in both groups were higher than 24 h after treatment,and the white blood cell count was lower than 24 h after treatment,and the observation group was better than the control group(P<0.05).Conclusion:Uterine hormone combined with cervical circleage and latex catheter balloon placement in the treatment of postpartum hemorrhage has a good effect,can improve hemostasis efficiency,improve coagulation function index and blood routine index,and promote postpartum recovery.
