摘 要:为减少医疗安全不良事件,鼓励医务人员从错误中学习,提高医疗质量,保障患者安全,总结某三级医院构建完整医疗安全不良事件管理体系的经验,包括制定医院医疗安全不良事件管理组织标准与制度标准,构建事件报告与处置流程,使用多种质量管理工具进行事件持续改进,做好事件处置结果反馈与宣传教育,落实事件提报、分析的考核激励方案等。经上述措施的实施,医院医疗安全不良事件自主提报件数显著上升,不良事件处置效率和持续改进意识也得到了提高。To mitigate medical safety adverse events,foster a culture of learning from errors among healthcare personnel,enhance medical quality,and guarantee patient safety,a tertiary hospital has developed a comprehensive medical safety adverse event management system,which includes formulation of organizational and system standards for hospital-based medical safety adverse event management,establishment of incident reporting and response process,application of diverse quality management tools for the continuous improvement of event handling,effective communication of feedback on the incident treatment results and providing education and publicity,and implementation of an evaluation and incentive scheme for incident reporting and analysis.Subsequent to the adoption of the aforementioned measures,there has been a marked rise in the volume of self-reported medical safety adverse events within the hospital,along with increased efficiency of adverse event resolution and an elevated awareness of continuous quality improvement among the staff.
