活血逐瘀清毒汤联合CRRT对脓毒症致急性肾损伤及心血管事件的影响

摘　　要：目的观察活血逐瘀清毒汤联合连续肾脏替代疗法(CRRT)对脓毒症致急性肾损伤患者疗效及降低心血管事件的有效性。方法共纳入脓毒症致急性肾损伤患者114例,随机分为对照组(57例)和观察组(57例),对照组予以西医常规治疗加CRRT;观察组则在对照组基础上予以活血逐瘀清毒汤。对比分析2组患者APACHEⅡ、SOFA评分、肾损坏标志物、心血管事件发生率以及炎症因子水平。结果治疗后,2组患者APACHEⅡ评分、肾损标志物水平、SOFA评分、心血管事件发生率以及炎症因子均显著降低(P<0.05),观察组各项指标明显低于对照组(P<0.05),且无较严重不良反应。结论活血逐瘀清毒汤联合CRRT对脓毒症致急性肾损伤的疗效显著,并可降低患者心血管事件的发生率,安全性良好。Objective To evaluate the efficacy of Huoxue Huayu Qingdu decoction combined with CRRT in reducing cardiovascular events in patients with sepsis-induced acute renal injury.Methods A total of 114 patients with acute renal injury induced by sepsis were divided into control group(n=57)and observation group(n=57).On the basis of the control group,the observation group was treated with Huoxue Huayu Qingdu decoction.APACHEⅡand SOFA scores,renal injury markers,incidence of cardiovascular events and levels of inflammatory factors were compared between the two groups.Methods APACHEⅡ,SOFA score,markers of renal damage(SCR,Cys C),incidence of cardiovascular events and inflammatory factors(hs-CRP,TNF-α,IL-6).Results the scores of APACHEⅡ,markers of renal impairment,SOFA,incidence of cardiovascular events and inflammatory factors were significantly decreased in both groups(P<0.05),in addition,the indexes in the observation group were significantly lower than those in the control group(P<0.05),and there were no serious adverse reactions.Conclusion Huoxue Zhuyu Qingdu decoction combined with CRRT has a significant effect on acute renal injury induced by sepsis,and can reduce the incidence of cardiovascular events,and has good safety.
