冠心病心绞痛痰瘀互结证治特点

摘　　要：目的通过临床流行病学调查方法探究冠心病心绞痛痰瘀互结证证治特点。方法选取辽宁中医药大学附属第二医院2020年4月—2023年12月的冠心病心绞痛住院病历,筛选出中医证候诊断为痰瘀互结及其兼证的病历,运用SPSS 26.0软件进行统计分析。结果冠心病心绞痛中医证型以痰瘀互结证、痰瘀互结兼气滞证最多见;四诊信息出现频率最高为胸闷、胸痛、舌质暗;用药频次最高药物依次是半夏、瓜蒌、薤白等,四气以温、寒为主,五味以苦、辛为主。结论基于临床流行病学调查结果,可知豁痰降气、活血祛瘀法是防治冠心病心绞痛痰瘀互结证的关键方法,瓜蒌薤白半夏汤作为温阳化瘀祛痰的经典代表方剂可广泛应用于临床。Objective With the help of clinical epidemiological investigation,to study the characteristics of syndrome and treatment of phlegm-stasis interaction in angina pectoris of coronary heart disease.Methods The inpatient medical records of angina pectoris of coronary heart disease in the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from April 2020 to December 2023 were selected,and the medical records of TCM syndrome diagnosis with phlegm and blood stasis interaction and its combined symptoms were screened out,and statistical analysis was performed by SPSS 26.0 software.Results The TCM syndromes of angina pectoris of coronary heart disease are the most common syndromes of phlegm and blood stasis,phlegm and blood stasis combined with qi stagnation.The highest frequency of the four diagnosis information was chest tightness,chest pain and dark tongue.The most frequently used drugs were Pinellia,Fructus trichosanthes,Allium stemon,etc.The four qi were mainly warm and cold,and the five tastes were mainly bitter and pungent.Conclusion Based on the results of clinical epidemiological investigation,it can be concluded that the methods of eliminating phlegm and qi,promoting blood circulation and removing blood stasis are the key methods to prevent and cure the syndrome of phlegm and blood stasis in angina pectoris of coronary heart disease.Gualou Xiebai Banxia decoction can be widely used in clinical practice as the classic prescription of warming yang,removing blood stasis and expelling phlegm.
