同期改良乳房上提术联合假体隆乳术矫正乳房下垂伴小乳症

摘　　要：目的探讨同期改良乳房上提术联合假体隆乳术治疗乳房下垂伴小乳症的临床效果。方法自2018年6月至2023年10月,重庆华美整形外科医院整形外科对49例中重度乳房下垂伴小乳症求美者,采用同期改良垂直瘢痕法乳房上提术联合假体隆乳术进行治疗。并对求美者满意度、乳房形态、切口愈合情况、近/远期并发症情况进行随访。结果49例求美者术后切口均一期愈合。所有求美者术后随访6~36个月,均未出现血肿、假体破裂、外露、感染等并发症。求美者对术后乳房大小及形态、手术切口瘢痕的满意度调查显示,非常满意39例;满意7例;不满意3例,经再次手术修复后满意。满意率为93.88%。结论同期改良垂直瘢痕法乳房上提术联合假体隆乳术,较分期行乳房上提术及假体隆乳术而言,求美者接受度较高,时间成本及经济负担较小。且术中应用改良垂直瘢痕法,术后切口愈合良好,并发症发生率较低,值得临床推广应用。Objective To explore the clinical effect of modified breast lift combined with prosthetic augmentation mammoplasty in the treatment of breast ptosis with micromastia.Methods From June 2018 to October 2023,49 patients with moderate and severe breast ptosis with micromastia were treated with modified vertical scar breast lift combined with prosthetic augmentation mammoplasty in Chongqing Huamei Plastic Surgery Hospital.Patients'satisfaction,breast shape,incision healing and short-term/long-term complications were followed up.Results The patients were followed up for 6 to 36 months.The incisions of 49 patients healed by the first intention.There were no complications such as hematoma,prosthesis rupture,exposure,infection and so on.The patient's satisfaction with the surgical incision and breast shape showed that 39 cases were very satisfied,7 cases were satisfied,and 3 cases were still not satisfied.The satisfaction rate is 93.88%.The 3 cases have been repaired by secondary operation finally.Conclusion Compared with staged breast lift and prosthetic augmentation mammoplasty,the modified vertical scar breast lift combined with prosthetic augmentation mammoplasty in the same period has higher acceptance,less time cost and less economic burden.With the application of the modified vertical scar method during operation,the incision healed well and the incidence of complications was low,which is worthy of further clinical research,application and promotion.
